Well as I continue to research my current condition I'm finding more and more that this problem that is in common here is also what I have. And after seeing many of you go to the doctor and find that nothing is wrong it points me to the conclusion that a yeast infection or candida is what this is. I myself have been away from sex for 2+ plus years I had fooled around for just a couple seconds before putting my condom on.
This in fact is just enough for the over population of the natural fungi/yeast that lives in both women and men and it can be contracted into the male sexual parts that quickly. Now I noticed the tingling in the tip of the head 12 - 15 hours after having sex. Eat plain yogurt (without sugar) the friendly bacteria inside yogurt will help balance this problem you see when you take your anti-biotics your killing alot of good friendly bacteria that will defend and push back the candida fungus. This is why after taking antibiotics it can come back even worse down the road. So you want to level your PH balance.

If you have the above mentioned symptoms AS WELL AS a tingling sensation when you have a BM then there is a good chance that it isn't epididymitis and it may in fact be prostatitis.
Fortunately most of the disorders that have the above mentioned symptoms ARE treatable with anti-biotics. The tricky part is for the doctor to figure out what it is exactly you have. Since they tests they run tend to be fairly inconclusive, the more information you can give them, the better. Tell them any symptom you have regardless of how trivial you may find it to be. The more information they have, the easier it is to knock things off the list.
I've been having this issue for almost 2 months and I first I was getting treated for "possible uti, possible STD, possible prostatitis", so they treated me with Cipro [anti-biotic] which is supposed to cover all of those. Some of the symptoms lessened but did not go away, I went in again, had another urine analysis done as well as a testicular examination and a prostate exam, that's when they decided what I have is "most likely" [yes, the actual words the doctor used] epididymo-orchitis.
I'm currently on day 3 of treatment but honestly feel no noticeable changes for the better or worse, which isn't the greatest sign, but I am on a 10 day treatment plan. I'll update this when I find out more.

I have had similar symptoms. I noticed an outbreak in the groin area before it started, and a bit of pain in my lower back on the left side. But since I was younger I was diagnosed with Warts/HPV, besides I have very sensitive skin and prone to rashes and occasional outbreaks since childhood. But I am somewhat confident that this might be Herpes. I was also tested for syphelous, goneria, and clymedia (spell check), and given my age (35) a lot of kidney/testicle symptoms for other kinds of ailments that effect the elderly are ruled out. I spoke with a number of others who have Herpes symptoms, and Herpes could effect some with continuous pain and others with very little. Also, sometimes you may have carried Herpes for a long time, I hear up to 8years, before you may have symptoms, and it is not in all cases that you have a break out. Some are carriers of the Herpes virus only and they do not know they have it. Plus unless you test the break out you don't know, especially if it only looks like a zit to a lot of people, I can imagine many people with strong immune systems might not even bother testing for it, and it goes away fast.
At least a couple of friends I know have told me that they didn't know and so they never bothered testing. One friend had only a tiny zit. But in all cases they describe varying kinds of muscular pain/pressure that moves around in the regions that everyone has explained in these posts. The key I hear is having stronger immune system, and boosting the vitamins you need (read up on herbs and over counter stuff for std's specially skin virus outbreaks). I started to take L-Lysine Aminos, which is an amino-acid chain specially assisting people with Herpes. My pains definitely got better. I have been asking around about Herpes. Someone told me after a few years of consistently cleaning up her diet, and getting nutritional/supplements from numerous alternative herbalists, while taking L-Lysine, her symptoms completely disappeared. She advised me against drug medication that suppress the symptoms, and said instead, you have to let your system purge it with cleaning up your diet, varying cleanses, and herbs. And she said antibiotics makes you weaker, so don't take it if you don't know what it is. My pain still comes and goes, but with L-Lysine it has gotten better. I am not a doctor or anything, but I do have a lot of friends in alternative health modalities, including various energy therapies. These are all suggestions. Even in my case I am still not sure what it is. I can't be sure it is Herpes. And I don't want to keep finding a label and focusing on the problem. My zits (which I suspected were zits), were gone quickly so I did not test them to see. The doctor then said, if I did not have outbreak, no use for testing. Plus, I used tea-tree and grape-fruit seed extract and they were gone really fast.
Here are some additional suggestions:
1- Frequent salt baths, with good himalayan salt rocks, will often cleanse skin pores and pull out toxins, especially skin related, including soothing muscular/joint pains... they are often used to cleanse the energy too.
2- Dr. Clark's main site describes the best cleanses, and the right order to begin cleansing your colon, and then other organs, in order to slowly absorb proper nutrients from your food.
3- Herbalists could familiarize you with muscle tests, and you can study them on your own, and see how much of what things (including herbs etc.) you need to take... The best muscle test I have found is when you use your whole body to test something. Hold the "thing" in front of your chest, on your heart, and ask your body to show you a "yes", then ask your body to show you a "no", while you relax standing. You will notice your body sway gently. Most people will experience forward motion with "yes" and back with "no", and when there is no answer affirmative or negative, then they rock gently side to side. I use this muscle test for many things, and I have become better at it over time, and really fast. Now, I can guess from 1-10 how affirmative my body will say "yes" or "no".
4- To boost your immune system you have to increase your metabolism, also in order to absorb more new nutrients.
5- Eating more greens, drinking good water with good alkaline and electrolytes, taking no sugars, no alcohol, and no coffee, is always helpful. When I have a clean diet everything is better. Drink natural drinks more... Coconut water is really good for you. Soda is poison. You can get fresh coconut, or get true bottled coconut water, which has a lot of electrolytes.
Lastly, I like how one of you here said "don't focus on the problem" too much. Focus on strengthening everything else, and remember your body can heal itself of anything. Invite more positive "stuff" all around you. There is enough proof that the body can kick any disease. Are there pains in the testicles and other places if this candida has gone on for three years? Like in the legs and feet with pain or in the eyes that make them turn red. I have been tested for all STD's and they just keep coming back negative each and every time. Have had an ultra sound of the testicles and doctor said that he saw where that the circulation may have been cut off for a time. I believe this is Herpes. That's it, I have been that soldier. No major symptoms, no blisters on the penis. Get tested, if you test positive for HSV1 and they tell you its only oral and have urethritis, it's genital.
Take Valtrex or the other one. You will feel better after a week.
