My son is 2 1/2 years old. He had hypospadias corrective surgery when he was 18 months. Now that I am potty training him, I noticed he had a urethral fistula right under the gland of his penis. I took him to the doc and got an appointment for surgery but now I am having mixed emotions about it. On one hand I do not want him to be self-conscious about it when he is older but do I honestly have to put him through the agony of another surgery?
What are the long term possible complications that may arise due to his fistula? Is it aesthetic or is it something that must be fixed to prevent fertility issues and such?
I want my boy 100% but I don't know if going about the surgery will necessarily give him that or not.

Due to trauma; occurs in males in which the urethra lies superficially near its end. A fistula may affect the discharge of semen from the normal meatus sufficiently to cause infertility.

Good evening sir. First let me say that I am very sorry to hear of the difficulty that your son is experiencing. I am 38 years old and have an identiical history to your sons. Though I am older and have to have major reconstruction of the urethra due to my age...I've recently postponed the surgery which was supposed to be next week due to the extent of the procedure and the aftereffects of it.
I had hypspadias surgery at the age of 4 which corrected the problem only for a short period and have lived with the fistula (in the exact same location of your son by the way). My parents, mother especially decided not to pursue more surgery due to the pain and discomfort I went through the first time.
My urgence to you is to have the surgery for your son. He is at the age now that he will not remember the procedure and will be able to live a relatively normal life after that. I "made do" over the last 34 years and am considering homeopathic exploration but for your son I would urge you to have it done. MUCH LESS risk is involved at his age and if he is not circumcised then the graft can come from the foreskin instead of the cheek which is what I needed. Have it done for him now when he won't remember it and save him years of UTIs and social embaressment that I endured growing up. I promise you that he will thank you for it later in life. Children's bodies and internal structures and considerably more resilient than ours and few weeks recovery and mild pain and discomfort is a small price to pay for a regular life over the course of his long life. By the way...my nephew had to undergo 4 surgeries before the age of 5 for the same problem and I always found myself wishing my parents were as attentive and dilligent as my brother and sister in law had been. The little guy is nearly 12 now and thank God has had no recurrences. It's worth it!!

Hi again. Sorry to bother you but wanted to answer your questions. It's not just aesthetic as it does impact the urethra as well as other issues (longer term of course) which could be kidney and bladder problems later in life .
It won't impact fertility as once he reaches puberty he will likely ejaculate through that site and his spouse can be inseminated after the sperm is cleaned through a centrifuge. So it develops more of a psychological difficulty often in one's self confidence and possible depression in later years. I hope this helps
