A little history...

About 3-4 years ago (age 18) I noticed a lump on or near my Epididymis (left testicle). It never hurt, and was never sore. A year later, randomly after long durations of walking my left testicle would hurt to the point of having to stop (rare but same pain each time). the lump never changed shape, size or density (firm and round). After a while I got curious of the properties of the lump and shined a bright light through my scrotum. The lump appears to be translucent or clear.

I have been to a doctor once for this issue; I was given antibiotics which didn’t do anything noticeable. Since then I've lost my medical ties to my parents and am working on getting things together to see a doctor. Though the random pain hasn’t returned, the lump still persists at the size, and same properties.

A few years later during a self check, I located a small round hard lump that doesn’t have a solid location in the scrotum, making it difficult to locate. It papers to be attached by some strand of tissue. It is a little larger than a B.B. and just as firm. This lump is not sore or tender, but it’s either new or only recently found (due to its inconsistent location).

I'm experiencing no discomfort or pain. I have a regular healthy sex life with no complications from the lumps. But its not very comforting knowing I have 2 lumps in my around my testicle. So any information on what they could be or advise on how to explain this to a doctor when that time comes (hopefully soon) would be helpful.

If both lumps seem to be attached by some strand of tissue, they could be spermatocele. These are small cysts on the epididymis that hurt only when they grow and are irritated by the cloths. Spermatocele are filled with the dead sperm cells and some milky fluid. They shouldn’t pose any threat or discomfort and I am not sure they would hurt as much as you explained unless the cloths irritated them during your walks.

I suppose that you do know that epididymis itself is a lumpy tube and that its lumps could be felt during a thorough search.

I still think that you should have these lumps checked as soon as you can and as for explaining, you could do it just like you explained to us.
