I know this post is old, but I couldn't help but notice the mention of joint pain (with alcohol) and liver problems. My sister has exactly the same thing and has been recently diagnosed with coeliac disease. Despite having seen a rheumatologist and a liver specialist, neither picked it up, and they both basically told her there was nothing wrong with her. It was only when she started getting horrible ulcers on her tongue that she was diagnosed by her GP.
She has only been gluten free for a short time now, so it is difficult to say whether the coeliac has been the cause of her joint pain, but I thought I'd throw it out there.

Gout is more likely to affect the big toe joints first before any other joing and I know this through experience of having gout myself, the whist and knuckles are one of the last joints for you to experience pain in from gout.
Alcohol will bring attacks on but not the major cause of it first starting either as I haven't been a heavy drinker even when young and had no more than around 6 pints or any wine or spirits in the last 10 years but still suffered badly at first until I found one of the main triggers that brings mine on, as long as I don't eat to much cauliflower mine is fine as that's a high purin veg.
