Codeine Containing Medication Survey

I am an MSc Health Psychology student conducting some much needed research into codeine use.  I need people who have recently used any form of medication containing codeine to participate (e.g. neurofen plus, co-codamol, tramadol with codeine).  All you have to do is complete this online questionnaire (which will take around 10 minutes), all responses are completely anonymous.  I really appreciate your time and support, I'm happy to answer any questions.
