The hymen is NOT right over the cervix, but at the opening to the vagina where it can easily be seen if present. In a virgin the hymen could have been previously broken in any one of several ways, such as "a bicycle accident" (this is why girl's bikes and boy's bikes are different). There can be some pain with first time intercourse, or none, depending on whether the hymen is completely intact, partially, or not at all. It can also depend on the bf being gentle, considerate and loving. Bleeding or not can depend mostly on the hymen being somewhat intact, or the 'time of the month.' Some females have been known to convince a bf she is a virgin, when not, by having first sex with him during her period. A bit of residual pain following first time intercourse, or even bleeding, is not totally uncommon, but should not last for more than a day or so.
