I'm 13, when I pull my for skin back there's a little piece of skin almost like a string attached from my for skin to my head (glans). 

What do I do?

Is it normal?

Do I need to see a doctor?

It's NORMAL and no need for a doctor.

This is very common until around age 16 or so.  Your foreskin separates from your glans and it takes some time for that to fully happen. 

Normal erections and masturbation will help it separate.  Don't force it, it could leave a scar.
