I am a 38 yr year old male, slightly overweight, in generally good health. I have been having upper abdominal discomfort/cramping that usually develops in the late evening in my upper abdomen just below the ribs, becoming more severe and radiating to my mid-back as the night progresses. It will usually wake me up in the early morning hours and not allow me to fall back asleep. At its height, the pain itself isn't necessary severe (I would say 5-7 on the 10 scale), but it is constant and unrelenting for hours which makes the pain/discomfort feel like a 10. It feels like my entire midsection just below the ribs is cramped up, with no position giving me any relief. About the only relief (although slight) is sitting in the tub with a hot shower on my back. It eventually subsides around daybreak and I can finally fall back asleep.

It first happened about 4 years ago after a day of spring cleaning, and drove me to the ER. They did a CAT scan which showed nothing and gave me intravenous Pepcid which apparently did nothing. After intravenous fluids and muscle relaxants, it subsided, but who knows what finally made it abate. They gave me no diagnosis other than cramping. It happened again about 2 years ago while on a long drive, but subsided before getting too severe. It then happened a couple months ago which again drove me to the ER. Did blood work and gave me intravenous fluids, and it subsided. Said it was cramping due to dehydration (I admittedly drink too much soda and not enough water, so it sounded right) and fatigue/stress. Gave me a Vicoden prescription for the pain if it happened again.

A few weeks ago we went to Disney for a week and it happened 3 time during the trip. The Vicoden helped a bit, but the discomfort still didn't let me sleep (so I was basically loopy and drowsy, but not able to sleep). Figured it was dehydration, although these times it seemed to also be associated with large meals. Since we've been back, it has happened two more times, but didn't necessarily seem to be associated with dehydration, fatigue, or large meals.

So now I am planning on going to my GP to really get into this. Possibilities seem to range from pancreatitis to gall bladder to IBS. Or maybe it is just dehydration cramping of the abdominal/diaphragm muscles. Has anyone had any experience with this kind of pain/discomfort in these scenarios that might give me some insight to a cause? Thanks!

I have muscle cramps on my right side, middle back. Cat scan said nothing. On Vicodin. Feels like my back is broken in middle of it. Getting more test on Tuesday. Everything you said, I have! c**p, it really hurts too.

Experiencing much the same.  My pain is in the center of my abdomen, below my sternum and above my belly button, but it often times will radiate and create a knot under the side of my ribcage on either side, or sometimes lower, where my appendix used to be.  The pain happens at night, and when it comes, I get severe mid-back pain, to the left and right of my spine.  This happened once before about 6 months ago, and the hospital found nothing.  Hospital trip this time turns up nothing.  In the past two months, I've had two bad colds that caused me to take lots n lots of cough meds & nasal decongestants, and also caused me to herniate a disc in my back from coughing during the night.  So that caused me to take narcotic pain killers and muscle relaxers...all of which I have since stopped.  Following that, I was on some more cold meds for a renewed cold, and then my bp spiked...so add BP meds and diuretics to the list.  I suspect the cocktail of meds has caused gastritis or an ulcer...but will discuss with gastroenterologist today and let you know.

Please let me know what this is when you find out. I have the same symptoms a couple times a year but have recently happened 3-4 times over the past couple months. It is so painful. I have not been able to get any real answers because it is kind of indescribable.

After suffering from this exact problem, which included a visit to A & E a friend explained the muscles attached to your diaphragm are linked to your ribs and therefore your back.  The sensation i had was that the spasms were so severe they were literally forcing the air from my lungs.  I was also having twinges in my back.  After reading these posts yesterday, I made an appointment at a chiropractors.  Having never been, I had no idea what to expect.  I explained my symptoms and that the doctor at the hospital had suspected gastritis, she laughed and got to work.  I must be honest, I did let out a sound that I could only liken to a dinosaur and that the sound of my spine cracking will stay with me forever!  After half an hour of this, I left feeling a bit shell shocked but guess what?......nothing since.  A slight twinge in my abdomen When i got up this morning but nothing else.  It's still early days i know but i felt I had to post this reply immediately because i really do "feel your pain".  It's worth a try!!!!

After having horrible cramping for last two years, studying this and trying different things, I find out a couple of interesting things.

1.  I love milk if I drink too much it can bring on cramps.

2.  Kids chewable vitamins and water helps

3.  I diet almost continuosly trying to eat less missing meals can bring on cramping session.

3.  I have an orange tree, and I find the thing that helps the most is I eat a 1-2 oranges per day or watermelon or banana it seems the cramps are much reduced.

4.  I am an engineer and can sit at my computer for hours without moving if I get into something very bad when I try to walk afterwards, cramping in stomach chest and back can put me to the ground.

Now if I dont eat wrong stuff, walk around a little 5-6 min walk several times a day, I can go weeks and longer without cramps.

I know exactly how you feel (I'm 43), mine sounds almost exactly like your symptoms. Occurs around bedtime or after falling asleep, boring pain at the xyphoid region, straight through to my back like cramping in my back. The pain the radiates to my throat and into the area under my tongue almost as if I bit into something sour but it's painful and is persistent. Tossing and turning, walking around the house, keeling over every now and then, until finally it subsides enough to get to sleep (probably from exhaustion). I've tried everything from Tums, Gaviscon, Zantac, Alka-Selzer, Prilosec (an OTC PPI), a new PPI Dexilant, peanut butter, Digest-zen topical oil, spoonfuls of peanut butter, warm milk and molasses, and peppermint tea (just to name a few remedies). Nothing but time seems to help. Hand an EGD which showed acute and chronic gastritis, but it seems to be something more intense than just GERD causing these symptoms. I think spicy foods, maybe beer and stress are my triggers, but it has happened without them as well. I feel like a gallbladder ultrasound and imaging of my pancreas are on the horizon, but don't know for sure. This has been going on for about 5-6 yrs now and getting more frequent, like 3 times in the last couple of weeks, but tonight is the worst! I feel full all the time even after small meals, have nausea and anorexia associated with food and have a little belly fat but overall in good shape. Don't drink excessively and never drink soda. No fast food, eat relatively healthy, organic nutrient dense foods, maybe have 2 beers in one night on occasion. What gives, the misery is awful!!!

From experience, everything in the body is connected with the spine, if you are subluxated anywhere on your spine it can cause severe issues in your organs, muscles, nerves and how they and your body as a whole function. I was going to go into have sugery on my shoulder for a separation that appeared on an x-ray, I did some PT for the problem and no results, finally went to a chiropractor and I have been pain free ever since. I prevented from having surgery through chiropractic care. I also believe you may find relief also from chiropractic. Make sure to ask people you know who use chiropractic care who they use, you don't want to just go anywhere. I have been experiencing a slight strain in my mid back that radiates to my abdomen while I sleep at night lately, which is bothersome because it feels like my stomach is having spasms and is painful. I have not yet been to see my chiro about this so this will be the thing I address next. I know I will see positive results. To add, sometimes muscle spasms that have no resolve with chiropractic or medication, many times having nutritional deficiencies can be the cause of muscle spasms and many doctors won't test for them, they will for Vitamin D, but anything else such as magnesium deficiency, Vitamin B deficiency, or food allergies such as celiac's disease (gluten allergy or intolerance) or other food allergies that could be the source of muscle spasm. If you have nothing else to lose, which if you've been through hoops to find some type of relief and haven't found any, you don't, I would recommend seeing a naturopathic physician, they have the same schooling as an MD and they even have more schooling because they have to have their PhD before entering school to become a Naturopath according to my research on ND's anyway, they really think outside the box of today's flawed healthcare delivery system. Seeing a Naturopath has really made a huge difference in my health. Be prepared to be treated the way doctors used to treat patients, with care, compassion and time, it may feel foreign in the beginning especially if you weren't alive or too young to remember doctor's ever doing that for their patients. Please don't mistake what I am saying about MD's as saying all MD's are aweful because I am sure there are a few out there still living up to the code of ethics but I haven't really seen any in my lifetime, and they have always given me the run around and a one size fits all answer to my problems instead of investigating, so that is why I am so pessimistic about them. I hope you find relief and the underline cause of your symptoms. Good luck!
