Dos anyone else notice strange particles in their urine?

No strange particles for me. Also to the previous poster I did have a bigger vein on left side of penis but no real pain at this point. Early on I had some random stabbing pains in the penis during the day but while that subsided the night time erections did not. It’s been about 5 years for me. Not one night without at least 2 erections waking up. Most nights it is upwards of 3-4. Not painful just uncomfortable. Goes away after urinating or standing.

Well to answer your first question, it all started with extremely foamy urine, after a month or two I remember seeing blood in my urine. After a couple of weeks, this condition started happening.no pain, just discomfort. Everything has somewhat gone down hill from there. I have lost sensation on the tip of my penis, I feel extremely tired during the day, mental confusion, anxiety etc (not sure if it's due to the lack of goodnight sleep). But as I mentioned before, I don't mind this condition, what worries me is the weird particles in my urine and loss of sensation of penis....
I am associating all or one of these causes for such a condition: extreme dehydration, bacterial infection or just severe anxiety...

Never had blood in urine at any point. Foamy urine may be something here and there but I would say extreme.

I have been taking Baclofen 10 mg and Clonozepam 1mg every night. They gave me 6 hours of sleep. But they are starting to lose their effectiveness. No sleep last night. Combined side effects: lose bowel movements, gas, minor knee pain, inability to focus at work.

I'm 100% with you on the increased gas and loose bowel movements (definitely still have a shape but they are more frequent and smaller than before). I once took a gas pill early on (been with this 5 years) that worked for some time in getting me 2-3 hours more sleep. No idea why that was helpful considering the symptom but it was. Inability to focus at work comes and goes depending on the sleep aspect of things.

I have been suffering from SRPE for 5 years. I am taking 1 mg Clonozepam, 10 mg Baclofen. It gave me 6 hours of sleep for about a year. Now suddenly it doesn't work anymore. I will take your advice and increase Baclofen to 15, and then to 20. And maybe reduce Klonozepam.

Actually this is a valid point that needs to be taken on board. The bizarre thing about this condition is what you eat and drink has a significant impact on the condition. In my case it was caused by Propecia, but interestingly if I eat rice with every evening meal this does alleviate the symptoms somewhat. EAT RICE! Don't eat spices or acidic things in the evening included stuff like trifles and berries or Ben & Jerry's ice-cream or dark chocolate. Putting a pillow between your legs also helps, but I can't really lie on my side. If I do I have to twist my hips and legs so there flat as if lying on my front. If you can lie on your front then go for it. Basically lying on my side triggers the erections. One more thing... raising one leg up to the side so your knee is just below your chest will also help. These aren't cures but they take the edge off. Eating rice makes a big different, it must settle the stomach and then the bladder somewhat who knows...

Did anybody try a neuorologist or a chiropractor for pudendal neuralgia, pelvic decompression and lower back mri? Did it help?

I went to a chiro because someone on this board said they got some relief by doing the spinal decompression machine at the chiro. The chiro did an x-ray of my lower back. I was diagnosed with spondylolisthesis, which means that the vertebrae are misaligned. The chiro thought the spondylolisthesis was causing a pinched nerve, which was why I was getting erections at night. I also saw a orthopedist and showed him the X-rays. He said go ahead and try the decompression machine, but he didn't think that was causing the problem. So I did the decompression for 4-5 weeks, 3 times a week. I thought at first it was helping but it turned out that it wasn't helping. that was disappointing because I thought I had finally found the source of the problem after 8 years of this. My next step is to see a neurologist. After that I will have exhausted all types of doctors. Unfortunately I think this is something I'm gonna have to live with.

What's so mysterious about this condition is that it ONLY happens while we sleep. If it were an infection, bladder problems, back problems, tumors, cancers etc etc, it would appear at random throughout the day. That is clearly not the case. Could it be that our nervous system has lost its regulation while we sleep? I think that we need to study more on how sleep regulates the body so that we can possibly find a cure or at least know the cause of what is happening to us.

Mine started after I took Lexapro for anxiety. Not painful, but erections 4-5 times a night waking me up, eventually led to complete lack of sleep. I tried many many medications for sleep. None worked. I am taking 1 mg Clonozepam and 10 mg Baclofen. It worked for a year by giving me 6 hours of sleep before an erection. Now it is losing its effectiveness and I am worried. There is nothing visibly wrong with my penis.
