A few more things I learned by trial and error:
Do not masturbate. It makes it worse, presumably because it irritates the nerves that are already damaged.
Do not change the dosage of Baclofen willy nilly. It makes it worse until your body adjusts to the new dosage. It is not a benign harmless medication.

I take back what I said about Baclofen losing its effectiveness. It may also be that people are experimenting with the dosage too much, and every time you change the dosage, it makes it worse, so they keep upping the dosage. I think that was happening to me. Stick with a dosage for some time to adjust to it.

Yes, but as a side effect of Baclofen. Over time, it got better. Now it is almost back to normal, except for excessive gas. You might be taking too much Baclofen. It is a nasty drug.

So I've been experiencing this issue for over 2 years to date.... with me I've been able to actually physically feel an abnormality around the left side of the head/gland, like it's felt as if something were out of place, plus I've had this tension going up through the thigh, leading to the penis as well, again all up through the left side. In my case I had had some kind of strange feeling (some itching, discomfort and the like) on the right side of the penis between glans and shaft for quite some time (several months actually) before the condition started. I sometimes still have it. This is one of the reasons why I think that this condition has some organic causes inside the penis. Some kind of inflammation. The reason that we wake up is that erections during the night are more prolonged (up to one hour) than having a normal sex-related erection. The reason Baclofen works is that it prevents erections (which is a well-known side effect).

Your reason for thinking that the cause of this problem is because the penis is inflamed does not answer the question as to why this condition only happens at night. Furthermore, if it were due to that, the erections would appear throughout the day randomly and not only while sleeping.

Yes, I also had (and still have) these sensations in my urethra when it started... so there really could be some connection!

My issue started to slowly come on off a very odd event where my penis was swollen but not erect. My wife and I are very active together so assumed maybe I got an injury. For about 3 days it would not return to normal form and erections were difficult. I was planning to see a urologist when after about a week everything went back to “normal.” I truly have no idea what happened to lead to that. I assumed maybe it was even an allergic reaction. Erection during incident time almost made my penis look like a coke bottle. Again that was like 5 years ago and never an issue like that again. A few weeks later it started to wake me once a night. Then I noticed that erections would stay on a lot longer after orgasm from sex. In some ways it was a newfound skill as I could keep having sex longer with my wife even after orgasm. Problem was that the night time stuff started more and as high as 5 times a night. I have managed without drugs hearing the lack of efficacy and only take ibuprofen before bed. Melatonin small dose as well here and there. I get up 2-3 times now which is manageable. Alcohol helps more with maybe one time waking up.
