Every year when it begins to get hot and I sweat, I break out all around the outside of my armpits. A few frequently look like ringworm. (round, raised circles), but some are just puffy red bumps. They get worse in the heat. Have treated with anti-fungal and alcohol to no effect.

That sounds unusual but we can rule out a fungus already since you have treated it and it's not actually ringworm or anything like that. I think it'd be a good idea to talk to your dermatologist and make sure that you even take pictures of it because it seems to me that this is an issue that bothers you a great deal. Keep me posted on what your doctor says.
