Sorry I'm late to this. I used to suffer from this problem a few years ago and had the problem for 5 years. Tried everything, numerous doctor visits and STD clinics. It is NOT sexually transmitted and its not an infection of any kind. But it is cureable.
What happens sometimes is the anaerobic bacteria can build up to unregular amounts under the foreskin and cause a 'fishy' odor. To test this, if you retract your foreskin overnight, the penis head wont smell the next day.
The only cure to this problem is to get circumcised. Nothing to be ashamed of. I had it done and it hasnt affected my life in any way whatsoever. Pleasure feels the same and if anything it actually looks better and feels much cleaner.
My doctor didn't listen to me and he kept saying I needed anti bacterial cream, but I needed to be circumcised all along!!

Furthermore to the above post. No medicine actually works for this problem as it isnt an infection in anyway. Hence why doctors cant tell you whats wrong. If it isnt an STD your best option is to get circumcised and the problem will go. I wish someone told me this sooner but I had to live with it for a few years.
