It is nothing you have done wrong, nor have you caused it in any way. The odds of this happening in the general population are about one in ten, which also means that about ten percent of all men have noticible varicosele.
Some are quite large,, and can cause pain. Some affect the fertility of the man. But most are just there, and cause no real problem. You may notice a slight difference in how low your scrotum hangs at various times, but other than that, there is no real appearance problem. The key is not to get overly concerned about this.
There are two types of surgery now to eliminate them, but neither is guaranteed, and most are not considered unless fertility is an issue OR chronic pain.
Two check to see if this is your condition, try these three things: 1) The distended blood vessles "feel like a bag of worms" to the touch, and give way if you attempt to feel them or squeeze them. This produces no pain, no feeling at all. 2) When you lay down and look for the same set of lumps and bumps, they seem to have disappeared, as the blood returns to the body. and 3) in a darkened room, a penlight held behind the scrotum to "transiluminate" the varicocele will show red, due to the blood, instead of clear or yellow (which might inidcate a hydrocele, or a bag filled with watery fluid.)
You can perform all three of these checks at home in the privacy of your bedroom. There are the same steps the doctor will repeat in the exam room to confirm this.

Yes, as it inidicates that you are entering puberty and maturing. The lowering of the testicles allows them to raise and lower to protect them, and to produce sperm at the proper temperature.
Don't worry about it, but wear an athletic supporter if it makes you feel more comfortable. Also, you could wear jockey shorts instead of boxers if it feels better.
It's possible that you have a torsion of the testicle.
This would require an urgent trip to the Emergency Room.
It is caused by a twisting of the testicle within the scrotum that might restrict or shut off the blood supply to the testicle.
It is important that this be check out right away.
If you've waited 22 hours, it's probably not a torsion, but you might try sitting in/soaking in a warm bath to southe the testicle.
But I still think you're headed to the doctor for some treatment... possibly an antibiotic and at least an examination.
