What's the difference in the costs of cortisone shots?

I have UH for health insurance I got a shot of Cortisone at Joint Specialist  $400. (Carpal-tunel) my son got a shot at Primary Doctor office in his Knee covered under copay $20 .

My Question: What's the difference?
