When I took it out, there was something on the head of my genitals

I am a guy nd i had sex severa times with my girl friend.neigther one of is bleeded.i felt dat my penis went in to her vagina.whn i took it out of it the head has come out of the skin.but the bond between head and the skin wasnot broken...i wonder y...and what is the solution for this thank you
