I have this also, first noticed about 6 years ago, small white shape and it has grown signifigcantly, I also have two spots on the side of my shaft and just under the head. It doesn't look like anything more than the pigment in the skin is completely gone.

White patches on the penis are either a) fungal infection b) vitiligo (it's more common than you think) c) scarring/irritation from improper masturbation or d) slight possibility that it could be related to a form of cancer or STD, but most STD's don't involve white patches of de-pigmentation.

It's hard to talk to a doctor about it because of the area it's in, but I've decided to go anyway, because no matter what it is, the sooner you go to the doctor, the better chance it can be corrected without causing any noticeable change to your penis. I was using KY Touch with warming for a while, and I think that may have caused my problem. Probably a reaction to the warming chemicals. But I also have vitiligo (diagnosed) above my eye, so I'm kind of leaning to the possibility of it being vitiligo. Problem with vitiligo is that it is random. It can show up, disappear, reapper, never go away, spread or not spread. Doctors only have theories as to what causes it (all they know is that it is probably an autoimmune disorder, where the melanocytes in your skin are damaged by either trauma or a chemical reaction, which causes a loss in pigmentation - hence the whiteness.) But the good news is about vitiligo is that it's perfectly harmless aside from being embarrassing.

If you're not comfortable talking about it with your primary doctor, make an appointment with a urologist or dermatologist, and have them diagnose it. They'll tell you exactly what it is, and prescribe the appropriate treatment for you. And, you'll never see them again, so you don't have to worry about being open with them about it. I'm probably going to do this because I don't feel comfortable talking to my female PCP about it. I've been a patient of hers for over 20 years, since I was a child. I'd rather see a stranger who I don't have to see again afterward, if I wish.
But, if you're with someone who notices the spots, and gets disgusted or otherwise freaked out by it, then just explain to them that it's just vitiligo, and it's pretty common. Or, if it's someone you don't plan to see again, but want to continue being intimate with for that night, just say it's a birthmark, most people will shrug and forget about it. However, if they persist to freak out, then dump em. Anyone who cares that much about superficial appearances of the genitalia are not worth being with. Find someone who actually has some compassion and who likes being with you for who you are, and who doesn't expect utter perfection.
In short, don't worry too much about it, but go get checked out by your doctor, or a urologist, or a dermatologist. The sooner you go, the better chance it can be corrected. If you wait 10 years, you may miss your window of treatment and have to deal with it the rest of your life, and run the risk of it spreading.

Guys, as many people in this thread have said already, it is most likely vitiligo. It is not as uncommon as some say, especially for men to get it on the shaft of their penis. It is harmless, can spread, and can be cured (to a certain extent) with various therapies. As for me, my mother has mild vitiligo and it is passed through genes. So if anyone in your family has it, even though they probably will not tell you they have it or even know what it is, you are at a higher risk of getting it as well.

I have vitiligo as described, on the penis, scrotum. It has now appeared around my eye too. I was told you are more likely to get it in these areas that have different skin from the rest of your body. (hands,feet,armpit,nipples, face and genitals). I have a skin biopsy to confirm it was vitiligo. Its more common that you think. 0.5% represents 35 million people
