It seems to have been over a year since this posted. I was searching for some information on this for my wife. These symptoms started for her a couple of years ago. We have narrowed it down to sweet Alcoholic drinks with Red Food coloring. Now we always ask if a drink has red food coloring or Grenadine (simple syrup and red food coloring) and if so, ask to substitute cranberry juice or pomegranate juice and it seems to work. One time she asked for a "Bahama Mama" with a cranberry substitute. It came back very red and I wanted to question the waitress, which I did not. She took a sip and had the reaction. It happened again last night. We asked about a drink and was told that it did not have red in it, but our friend got a drink that was red and she wanted to try it. I told her we did not ask about that one. Turns out it did have it. Needless to say, she had a reaction So we stay away from red food coloring and alcohol.
As far as red wine goes, she does drink some, but very little and no reaction. I have a friend who had the reaction with red wine and it turned out to be a gallbladder issue. Once it was removed, she did not have the issue any longer.
Ironically, My wife has had a couple of tests on her gallbladder and it comes back as low, but within a normal range. Removal could be a solution. I had mine removed for pain and spasms I was having in the front upper right abdomen. I continue to have a slight uncomfortable feeling where it resides, so I recommended to her not to do it.
We think it is all attributed to red food coloring/dye and Alcohol. There are some articles that support this, but the gallbladder seems to be common as well.

Right upper back issues are related to gallbladder, so issues are with liver and gallbladder. If ok with medications consider alpha lipoic acid and milk thistle

Hi! I know this is an old post but Im just wondering if you have ever figured out what the cause was? I get EXACTLY the same thing when I drink anything alcoholic that is fairly sweet. Its the worst and I have no idea what it is. My Drs all look at me as if Im crazy when I ask them about it.

I have/had IDENTICAL problem. I don't drink anymore but I used to heavy.
First drink or two been my shoulders and around my spine a bad aching feeling.
I got allergy tests and wallah I'm allergic to barley. Definitely could be a reaction.

I have a similar upper back pain only when I drink whisky. After a couple shots, I would get the pain on one spot—by the right shoulder blade, which then radiates to the right side of the neck. It usually lasts only for 15 minutes, then it disappears. Anyone knows what this is? Thanks

Pancreas is the problem I am sure of I have same pain I go buy milk thistle and cranberry and never put the alcohol down because you clearly would not be asking this if for one minute you would have to quit drinking because all in all the the real answer. Thank you for your time ...Leann
