7days after holep, I am having Frequent urination at the rate of 1 pee per hour making it difficult to have any sleep. Just wondering if anyone had the same problem and how long it lasted.

For small prostate, expect retention and recatherization after HoLEP.

“Some patients, particularly those with small prostate glands, are unable to pass urine all after the operation due to temporary swelling of the prostate area. If this should happen, we normally pass a catheter again to allow the swelling to resolve and the bladder to regain its function. Usually, patients who require re-catheterisation go home with a catheter in place and then return within a week for a second catheter removal which is successful in almost all cases.”

Frequent urination may take several months to get better:

“Some of your symptoms, especially frequency, urgency and getting up at night to pass urine, may not improve for several months because these are often due to bladder over-activity (which takes time to resolve after prostate surgery) rather than prostate blockage. Since a large portion of prostate tissue is removing with the laser technique, there may be some temporary loss of urinary control until your pelvic floor muscles strengthen and recover.”

Hi have been off line for a five day road trip.

Dr. Krambeck was very aggressive, taking about 90% of my bladder neck area. As for the surgery time it was an hour including morcellation time. I had doing my Kegels for about a month before the operation and I still do them but not on a daily basis.

IMHO, the greatest factor for incontinence is the percentage of the bladder neck area.

I agree I am envious of your continence.

I have been doing well even at the gym. Occasionally I will feel a single drop but nothing to requires a pad.

With that said I have had two occasions, a week apart, where I woke from a deep sleep and my underwear was wet to the point of dripping. Yesterday my wife and I stopped for lunch and had some sweet tea. Usually it doesn’t bother me but this must have been really strong, 45 minutes down the road I was in agony and then I made several more stops.

your and Blayneb, who had done Kegel for one month to strengthen the external sphincter before HoLEP, but 9 months after HoLEP are still suffering incontinence.

The fact that for both of u the degree of incontience decreases over time suggests the internal sphincter is slowly regrowing as it shouldn’t take that long to strengthen the external sphincter.

Hi DL, I remember that it was about that for me. It does get better. I remember trying to retrain my bladder by waiting an extra 15 minutes.

Yes, I’m not a doctor but it appears that both Dr. Miller’s patients and Dr. Das patients have much shorter periods of incontinence.

I had a rather a small prostate 40 gram or so, much much smaller than 215g of urs and 150g of Blayneb’s.

the correct way to look at it is that u both had selected the ‘perfect” procedure to correct the the situation by reducing the prostate size. Except the incontinence which is diminishing, both of your lives have become normal. That is a great outcome. I had read on patient.info that people who had failed RESUM, PAE, UROLIFT.....their lives were miserable. Some didn’t know what to do next. Had u both chosen another procedure, the outcome might not be that good.

About rare occasional outbursts of incontinence, it could be the healing process is still not complete. Spasms or something triggers it. I would give it more time to heal and would try to recreate the situation to see if u could find out what triggered the occurrence. Deep sleep could due to stressing event in a dream .

I am seeing a new doctor and I intend to see if I can get some new imaging.

I believe there are a lot of nerves in the prostate and it takes longer time for nerves to heal. That might be one reason for the urgency incontinence to resolve itself?

215g is exceeding large prostate. If I were u and Blayneb, I would do TRUS every year or 6 months interval to check the prostate size and plot it out vs years. This will let u know if the prostate is growing fast and into the danger zone.

If insurance pays for it, i would also do yearly urodynamic sand cystoscope to make that the prostate and bladder are health. It is better safe than sorry. Size alone does not tell if one has BPH. I had 40cc and was suffering severe BPH.

I know of no diagnosis that will tell the health of the nerves.

As for the sudden episodes I will just and wait and see. Regarding the tea I just have to be smarter. It was VERY STRONG and I drank a lot. It was SO GOOD but ther reaction was strong and quick.

I hope you are doing well. I am sure that you will see improvement. Another factor in healing is age. I am 73 and I can guarantee that takes longer to heal now than when I was in my 50s or 60s.

It seems ironic that we have surgery so we can urinate and now we can’t stop, at least on a temporary basis.

Glad u found out tea stimulant was the culprit.

QoL is the best judge. If u and Blayneb feel that the quality of life has been much improved then u both had picked the best procedure to resolve the BPH problem.
