I know that the shield is overkill but when I am doing back exercises that push straight down or obliques I have leaked. There is now time to get to a urinal. My doctor removed 90% of my bladder sphincter so it will take time to return that to full function, if ever.

I had watched several YouTube HoLEP videos in which they showed that the overgrown tissue inside the prostate was cut into 3 pieces, then each was pushed to the edge of the bladder neck, then cut at the edge and let it fell into the bladder that is the internal sphincter was completely removed. So my impression is that in most HoLEP, the internal sphincter is completely removed, yet clinical trials results showed that 80% of patients had recovered from incontinence in 3 months (my memory is vague), the rest 20% also recovered within 12 months.

Sorry to hear this. It could be scar tissue has formed.

I met a man from Canada that had a TURP. He over did it in the gym with a spinning class and weigh lifting. He went into urinary retention. So the doctor had to do the TURP. again.

Don’t understand why weight lifting will cause blockage and need another TURP. Have time to explain?

My surgeon said she took 90%. She als said her friend, Dr. Nicole Miller, removes about 65% of the bladder sphincter. So it seems do vary according to the doctor and possibly on a case by case basis.

It can cause the remaining prostate to compress around the sphincter. It likely the spinning class was the primary factor. He is from Canada and may not have received adequate post op instructions. We are told to stay off bikes, don’t ride horses and watch out about constipation. He was in the spinning (bike riding) class for an hour a day, 5 days a week. He said the instructor had them standing and sitting alternately. This puts a LOT of PRESSURE on the prostate. Especially when you think about how small those seats are.

Last year before I went back to lifting, Dr. K’s nurse advised me to go very easy. Again heavy lifting can cause damage to a very tender area. This fellow was trying to lift the same weight that he had before his surgery.

I don’t usually wear a shield for every day activities. I wear the shield at the gym because of the potential chance for leakage. I still do my kegels to help strengthen my outer sphincter.

“It can cause the remaining prostate to compress around the sphincter.” I can understand during the prostate trying to heal period, applying pressure to the prostate such as straining or lifting weight can deform the prostate and causes it to bleed and as a consequence, it will take longer to heal. However, I still don’t understand why it can cause new blockage to form.

I can’t answer that, I am not a doctor. I was told not to do the things I mentioned. My friend did do those things and he had to have his prostate scraped out. Those are his words.

A consequence for creating new tears, injuries or wounds on the prostate or bladderneck could be it increases the chance of scar formation. Scar is what causes new blockage.

I probably am the most unlucky person on this thread. Just visited a local urologist, who measured my Qmax and it was 2cc/sec, which is a factor of 20 lower than 6 weeks ago.

There is no doubt that after holep I have scar in the form of either bladderneck or urethra blockage. It this is not the case, then the bladder pressure will be a suspect.

Yes, I will be going back to Dr. Das to do a cystoscopy and hopefully he could clear my blockage. For the urethra, tubes with increasing diameters are used to break open the scar on urethra. For the bladderneck, a bladderneck incision is sued to cut open the scar.

The bad thing is that scar could come back.
