Hi Just Me. Thanks for the advice. I actually had a cystoscopy yesterday morning [my first one in 11 years] and my local urologist said that I have a vesicle neck contracture which is causing the obstruction and that, if it does show up after HOLEP, between 3-6 months after HOLEP is when you see it more than any other time. I'm 4 and a half months out since my surgery. He advised me to see Dr. Lingeman to have the procedure and I called and spoke to one of Dr. Lingeman's nurses and set up surgery for sometime next week. The nurse told me that it was about a 20 minute procedure under local sedation rather than general anesthesia and that, as long as I can urinate after the procedure I am free to go without a catheter. I was told that normally this will take care of it but it's theoretically possible that it will have to be repeated but it's not likely to happen. Again, thanks very much. Jack.

Glad to hear it is getting sorted out. I have the highest respect for Dr Lingeman. If it can be fixed He is most likely the one to do it. Keep us posted .In my prayers just me

Good to hear that Jack is getting help; will be praying for positive outcome.

I am now 14 weeks post-op and close to attempting going "guard-less".  At time it has been difficult to track the improvements with incontinence as I sit a fair amount of time at work.  Over the weekend I was able to run errands and put in the garden without changing oa guard.

Hi big 500 , If you have any doubts ,give the next size down a try , They are for drips and dribbles. I still have to use a little something just in case. My job can be quite strenuous at times. Glad to hear everyone is doing well . Are there any regrets from having this surgery? just me

No regrets at all.  So far HoLEP has given me my life back

Just checking in as a new poster. I'm a 69-year old white male (obviously). I consider myself pretty healthy except for BPH. I've had a steadily elevating PSA for about 15 years, started at 4, now 12. Three biopsies fortunately all negative. My signs of BPH include having to get up 3 or more times nightly, urgency, difficulty emptying my bladder if my bladder is too full, poor stream, and usually only being able to urinate 3-4 ounces at a time (not sure if that's all my bladder will hold or whether that's incomplete emptying). I've also needed Viagra for the past several years. My prostate measured 93 ml at the last ultrasound. I found out about HoLEP on this forum and have the procedure scheduled with Dr Lingeman in Indy at the end of the month.

What I'm hoping for is rapid recovery and elimination of symptoms, no long term incontinence, no increased impotence, and no post-op complications. One poster said that his impotence improved after his HoLEP--that would be icing on the cake.

This forum was helpful to me in deciding to have HoLEP, and I thought I would give back by documenting my experiences. I'll post again at the end of the month--wish me luck!

Hi Larry! Thanks for joining us and speaklng up. I'm sure that the surgery will eliminate much of your issues. I haven't mentioned this here before but in last 6-8 months (2 1/2 years post op) I have had moments of needing to find a place to urinate on short notice but I think that happens to all. Maybe once a month or less (I drink a lot of coffee). But I have no regrets and wish more folks knew about it. My DENTIST told me when I told him I had a urodynamics test (the most embarrassing - let alone painful med test i ever had). He used Dr. Das right here in Philly and I'm glad I found him.

You've hear other experiences here - and thanks for sharing yours as you go through it - so you've seen that no one has not benefited - in some way - from it. If you plan for a 5 month fully recovery period (may be less) yiu'll be fine and have many years of restful sleep too - let alone seeing how far you can urinate.

Jack here. As I mentioned a few days ago, after having HOLEP surgery with Dr. LIngeman on Dec. 12, 2014, I came down with a UTI in March and since then, I had frequency, some incontinence and finally about three weeks ago my stream ended up being a very slow dribble with no real stream visible. I saw my local urologist in Ann Arbor, who, like Dr. LIngeman is tops in his field. He did a cystoscopy last week and said I had a vesicle neck contracture which is, I believe, a form of bladder neck contracture. There was a stricture at the bladder neck. I went to see Dr. Lingeman yesterday and he made three laser cuts to relieve the obstruction. I drove back today and had a full stream for the first time in a couple months. I had no pain today from the surgery or any burning while urinating so I'm a pretty happy camper today. I wasn't excited about having to have a second surgery but I understand that strictures and bladder neck contractures happen sometimes after surgery for enlarged prostates, even under the best of circumstances, and Dr. Lingeman is a great surgeon, in my opinion [echoed by many others on this site. It was only 10-15 minutes of actual surgery time, I believe. I'm keeping my fingers crossed things hold up and will report as time goes on. Jack.

Hi Steve, I have been reading all of these post as I'm contemplating this surgery. I've learned that IBS is diet related & my wife's IBS is almost gone since we changed our diets.  Best wishes and good luck, Stan

Stan there are many reasons that cause IBS. Food is one, anxiety is another. I spent years with a Gastro guy and was getting colonoscopys (more than needed) and a friend suggested a medical psychiatrist. He put me on adivan and it worked. I take only be flights or high stress days and it works! In my case it was not the diet. If you have the BPH issues the others here have, I think yiu'll be happy with the results. The big issue is where is the nearest qualified surgeon. We have Dr. Das here in Philly but - to my knowledge no one else in the Mid-Atlantic states. This is still so new - not that new, but relatively, that most urologists have not heard of it.
