"Extensive PTB" means Extensive pulmonary tuberculosis. You have TB. "Irregular hazed infiltrates..." describes what they're seeing on the x-ray. It's spreading and the "cavitation" is not a good sign.
Your doctors should be treating you. Antibiotics is the usual course of treatment. I urge you not to delay treating this either.
