I am worried about my sperms as i am failing to impregnate my wife they are cream whitish but with thick yellow things like jelly. Is that means i have some kind of a disease

It could mean you are dehydrated.
You will need a semen analysis to find out if there is a problem. Color is not an indicator of your fertility.
