I always avoided public toilets, I'm 20 years old, and since may 2009 found it hard urinating in general even at home, I've been to urologists since, he said I had a distended bladder, I had several invasive procedures done there's no blockages my bladder is fine urethra has no problems.. then I went back in february they said my bladder isn't distended anymore and discharged me.

Since then I've still been struggling even at home especially when family are around the toilet is close to kitchen so I can always hear noises, sister sits in room next to toilet on computer which is even more of an inconvenience so I think it's anxiety and a mixture of other things that is a confidence thing too, when I was 18 or lower it wouldn't even by on my mind if anyone was in the room next to me or could go in public easy it started around May of 2009, my life is now based around this problem my attendance at college is suffering because of this, my social life especially, I know it could be Paruresis but is there anything else it could be?

I have heard about people who can not urinate unless some conditions are met. in your case those conditions probably include quiet.
maybe you should try bringing some earplugs or alike, see if it helps.. atleast for a start.
