Hi, I'm 25 years old and for the past month I've been getting this sharp pain on the inside part of my left testicle when ejaculating. There's no pain in the build up, only while I'm ejaculating and the second I stop, the pain goes.
But after it can become a bit sore for the next day if I sit in the wrong position or if I walk and my penis bumps against my balls (I wear boxers)
I think it might have happened when I was with this girl and she was playing with my balls. I had to tell her to stop because it was getting rough.
Anyways since then I've been to see a doctor and he gave me 10 days worth of antibiotics as he thought it might be an infection, he could'nt feel anything wrong with them.
I did notice that the left testicle was sat back and down more than usual. I did not masterbate or anything for 4 days, then a whole week while I took the antibiotics and the testicle has moved back to its normal position in that time.
I finished the antibiotics and my week of celibacy and day by day the nagging pain went away until I could not feel anything more wrong with it.
So for the last 3 days I have masterbated a few times but the sharp pain while I ejaculate is coming back. Its getting annoying now and can not find any desciptions in wiki or medical forums that fit my symptoms exactly.
There's no blood in my urine or semen and it all seems normal there just the damn sharp pain when I ejaculate and some soreness afterwards.
This happened to me before about 2 years ago in the same testicle but in a different part (around the bottom, not the inside part of the front) It sagged alot more that time and I waited it out for a few months and it went back to normal. But that time the pain during ejaculation went away within 3 weeks but the nagging, sagging testicle stuck around for a few months.
This time its the opposite, the pain during ejaculation is a bit sharper and has stuck around longer but the sagging testicle has already gone.
Does anyone know what this is?

It sounds as if you may have some irritation of the vas deferens or collecting ducts that the semen travels through but not urine itself. The location is likely close to the point that the seminal ducts connect to the main urethra since you continue to have pain with urination afterwards.
An infection such as epididymitis is possible as well.
I would recommend that you see a doctor to be evaluated for an infection that may need treated with antibiotics.

But I dont have any pain with urination, just ejaculation.
