I have an interview coming up at a hospital that will do a urine nicotine test ... I'm wondering if anyone had problems smoking at a job they DID get hired at if they started smoking again. For instance this hospital doesn't require existing employees to be tested, only new hires. So once if I pass the test, and get the job, I'm then an employee, right? I wouldn't try to use semantics or BS them if I were caught/reprimanded or anything, I'm just curious if anyone here had problems smoking on the job once they passed the initial test. Smelling of smoke, seen smoking/with cigarettes, etc.

The questions you are asking are particular to that employer.  Only they can answer "what if" questions concerning their policies.

Seriously though, you're in health care.  You know the risks of smoking so just quit.  I guarantee that even though you think nobody can tell a non-smoker will know you smoke.  Your hair, clothes, everything will stink.

Think of the money you'll save too.
