Corns are unsightly, painful and not just the province of older people. Unfortunately they are more easily prevented than cured. Home treatments are generally ineffective and can be dangerous, but a podiatrist can offer quick and painless treatment.

This painful foot complaint is nothing new : they are believed to have been described as early as 300BC by Hippocrates, who was credited as being the ‘father’ of medicine. And the predecessors of modern podiatrists and chiropodists (which is a UK term) were ‘corn cutters’ who toured around their locality, offering relief (with the forerunner of the scalpel) in the streets, markets and even at fairs.

What exactly is a "corn" and what do you need to know about it and how to treat them?

What is a corn?

You may have been lucky enough never to have had one, or if you have, possibly you didn’t recognize it for what it is. What do they look like? Well, they generally appear as a circular patch of hard skin, sometimes with a visible depression or elevation right in the middle of them. However, some can just appear as a small yellowish or white circle (‘seed corns’).

The skin around the corn may be white and moist – particularly when they appear between the toes – or they could be red and inflamed.

Although not visible to the human eye, they have a hard core which is an upside-down cone, meaning that the tip of the cone points inwards.

This is one of the reasons that they are so notoriously painful as whenever they are pressed that point ‘digs’ in, causing pain.

Where do you get them?

The most common sites for them to appear on the foot are on top of toe joints and on the sole of the foot - particularly over the ball of the foot – although they can occur anywhere on the foot, these are the most common spots. As mentioned, certain types of corn can even appear between the toes, though it's not as common.

What causes them?

If you've had corn before you may have wondered where it came from and how? They are basically caused by friction – which is mainly caused from wearing shoes. The skin of the foot is subject to differing types of pressure and friction such as shear or compression, and this is thought to account for all the different types of corn which can be seen to appear on the foot. 

The most common type of corn is the hard corn which is also known as heloma durum. The soft types of corn which commonly occur between the toes, however, are called heloma molle.

Hard skin (callus) and corns form on the foot directly in response to pressure and friction, specifically in order to protect not only the skin but the underlying structures as well. 

Although tiny, they are generally very painful and in the case of people with diabetes, can even be very destructive.

Who gets corns?

Many people mistakenly think of them as only affecting older people and there are many jokes about old ladies and their corns. But although they are less common in children than in adults, they can (and do) affect people of any age. No matter your age, you are at risk of getting corns.

Can corns be successfully treated?

They can easily be treated, but are difficult to cure: meaning that they are extremely likely to come back again. It is thought that the means by which the skin forms them is very complex, involving growth factors, and the skin retains a memory for forming them, so just does it again – and again

Also, if the cause of the corns is not removed – pressure and friction - it is obvious that the same thing will happen and they form again. So prevention is better than seeking a cure.

How can they be prevented?

Fashion shoes have a lot to answer for. You only have to look at the shape of the toes of some shoes – narrow and pointed and not at all like the shape of foot – to realise that they are going to rub on the tops of toes, as well as rubbing toes together.  

Over time - and in susceptible individuals - this will cause corns on the tops of bony toe joints, and between the toes.

High heels tip the foot down into the toes of the shoe, again cramming them together.  Tight shoes (as high-heeled court shoes have to be in order to stay on) will rub the sides and heel of the foot, potentially leading to corns.

But fashion shoes are not the only culprits – any poorly-fitting or tight shoes can rub the feet.

So make sure your everyday shoes are a good, but not tight fit and reserve high-heels and ‘winkle-pickers’ for special occasions only.

It should be said that some people will still develop corns despite good footwear, as their walking pattern is not ideal and leads to uneven distribution of pressure.  The areas of excessive pressure will develop hard skin and corns – such as the balls and sides of the feet.

What is the treatment?

The best way of treating corns is to have them removed by a professional foot specialist – a podiatrist.  

It looks scary as they will use a scalpel to cut out the hard core (or nucleus) of the corn, but is almost always a painless, bloodless procedure.

 At the same time they will identify the cause of the corns and how to avoid them.

Can you treat corns yourself?

A lot of money is made from the sale of home treatments for corns. Creams, plasters and lotions containing keratolytics (‘skin dissolvers’ or ‘caustics’) like salicylic acid, will certainly soften the area around the nucleus of the corn, but are unlikely to enable you to remove that hard centre.  Also because they are very non-specific, they damage the skin generally – particularly the softer normal skin.  This can be particularly dangerous in people with diabetes who may go on to develop ulcers as a result of such damage.

Corn ‘knives’ sold for use at home are also not a good idea, because of their potential for damage of normal skin.  

They also plane skin off parallel to the foot which is ineffective for corn removal as they grow into the foot and need a special technique to cut them out.

Again, corn knives should never be used by anyone with diabetes, because of the potential to cause a wound, which may be difficult to heal and become infected.

Some people suffer with exceptionally painful corns, which they just cannot get rid of and find treatment very painful.  These are known as neurovascular corns and contain nerves and blood vessels, which make them so painful and hard to treat. Fortunately they are not very common and podiatrists have special techniques for their removal involving use of an electrical current (electrosurgery).
