Seems I am late to the party on this discussion. I am 44yrs old & apparently in the peri-menopausal period. Sadly I only began having orgasms at 40 (yup, after 19yrs marriage & 4 babies). This was thanks to my sister buying me a wonderful Zumio clitoral stimulator device. I have learnt so much about my body in the past 4yrs including having had internal Physio body work & pelvic floor (pubococcygeous muscle) release. I have only just begun having not only anal/rectal pain after quick & intense orgasm, but tonight it included gut cramping & actually did need to pass poo. It came on about 5mins after orgasm & took about 20mins for the rectal pain to subside (yup, whilst in agony on the loo). Whilst I have read in posts that Drs say the cause & cure is unknown, I have a pretty good idea it is caused from a strong oxytocin hormone surge (I am a midwife too). When oxytocin is released from the brain in pulsations during orgasm it causes strong smooth muscle contractions. In orgasm, this causes the vaginal wall smooth muscle contractions which encourage sperm to travel up to the fallopian tubes to find the ovum. Oxytocin is the hormone of LOVE, connection, & reproduction. It is also the hormone of birth. To get the uterus to contract (smooth muscles) the brain must release oxytocin (& if it doesn’t our obstetric care starts are synthetic artificial version to make the smooth muscles contract). The gut & anus also contain smooth muscle (as opposed to skeletal muscles in your arms, legs, gluteals etc). This means that when the smooth muscles are stimulated to contract through oxytocin release, it might cause the sensation of gut cramps which flows through to the rectal muscles also. To stop a woman labouring during birth, it’s important to override the oxytocin. This can be done by a smooth muscle relaxant e.g. salbutamol or terbutaline (some meds as used to relax the smooth muscles of the lower airways which constrict in an asthma attack). You do need a prescription for terbutaline but in Australia a salbutamol (ventolin) metered dose inhaler (puffer) can be bought over the counter at a Pharmacy. Many woman who have painful Braxton Hicks practice contractions in pregnancy use a salbutamol puffer to calm down these smooth muscle contractions of the uterus. So it’s quite effective & would very likely work to relax contracted anal muscles. The other not so pleasant technique would be to somehow kick in your adrenaline response (fight/flight response). Because adrenaline counteracts the hormone of love & shuts off oxytocin release (which comes with your relaxation response from the parasympathetic nervous system). So hence why pacing around or doing exercise might work.
