masterbation problems

I AM 24...i started masterbation in the age of 12.more than 10 years i done..now my hair started fall ..it has little baldness in front of head.little big pain..my pennis is so weak...if it erects it wnt withstand for more ..might be withstand for some seconds..it fall down quickly...my sperms is releasing very soon in seconds..i stopped smoking before 4 months.doing masterbation monthly twice..i am scared about my life..now onwards i am completely stopping masterbation..what is the permanent cure for my diease.
