my son complains regularly throughout the day of pain in his penis. i took him to a physician who said nothing was seemingly wrong, and they did a urinalysis which was negative. he is not pottytrained and i put underpants on him to observe if the pain was happening at urination. it is not, but i have observed he has an erection when he is complaining. i don't know how to help him and he cries about it often. Have you reported erection pain to the doctor who examined him? Has he considered an ultrasound to have a look at the genital organs?
