Well, one way to tell (though most won't notice in the heat of the moment) is that when she DOES have an orgasm, her muscles will start to contract, especially her vagina. There are also smaller signs of intense arousal, such as heavy breathing, her face and neck and such getting flush. Not all girls squirt, so that's not really a good way to tell. Just listen and pay attention to her body language.

Yes, if she's getting wet, then at some point, she has been turned out.
