what could this be?
This must be an inflamed hair follicle. I often get these! Although it is not advisable to squeeze it and touch it, I usually put warm compressions on it, so that the puss can come out and usually a hair pops out as well. It is said that these lumps should be left alone but I never do because I know they are inflamed hairs.

However, it may also be a cyst filled with fluid. Don’t touch it in that case.

But if the white stuff is more yellow in color, then it is an inflamed hair. Mine (lumps) are usually not that hard. Do you shave down there?

If you decide to visit your doctor, he will examine all sides of the scrotum by having your flex leg on side being examined to increase access to area. Probably he will lift the scrotum to inspect visually the posterior side. He needs to examine also the scrotal sac by rolling the skin between the fingers of one or both hands. Gentle palpation should diagnose common problems such as a hydrocele, varicocele, testicular lesions, and inguinal hernias, as well as your problem of hard white lump. I have to tell you that any swelling within the scrotum should be transilluminated for further evaluation. Testes should feel smooth and glassy in texture, note size of 4 cm, shape, and symmetry. Doctor needs to examine individually each testicle, between his fingers of one hand for lumps, nodules, masses, or tenderness. It is important that he avoid excessive pressure during palpation, which could cause a deep aching sensation for the patient. He must feel your hard white lump, if that is on the front or the side of the testicle it is usually painless. If you have any lumps or other symptoms, it does not necessarily mean you have testicular cancer, but I would recommend you to be checked by a physician.

i have the same white hard lump in my scrotum. it had been here for almost 5 years. until it becomes i think about 1mm in diameter.
until i decided to cut it and to my amazement, it was really hard like a peeble, white and hard. i just pressed the skin to prevent feeling the pain when cutting, then i pushed outside the white hard substance using the tip of the knife. it sticked to the skin. it was removed whole and when cracked was powdery like limestone. whatever that was, i was relieved it was removed, and my scrotum is now free of that lump.

I am 38, I also have a similar experience as you, I have observed the same lump on my scrotum for over 5 years now, not painful but usually uncontrollably itchy. At first it was just one lump but now has multiplied to more than 15 and of varying sizes (some as small as pimples others a bit large as pea size). Some times they get very small and other times they get a bit larger and really annoyingly itchy. Once I painfully cut one open (not without bleeding though) the lump was creamy-white in colour and like Stones attached to the skin. Over time more have become swollen and ripen and soft, at which instance I have cut it open and pressed out the very thick creamy/yellowish minute granular fluid (it's like non-smoothly grounded sand particles) until it empties with considerable bleeding. When that portion of the scotum heals, it becomes normal again. My worry however, is that it has multiplied over the years and It gets me really worried. An advice would really help me. Thanks
