Had HoLEP on Prostate of 231 grams 7/14/2021 with a left side Inguinal Hernia. Recovery at 4 months is great from HoLEP.

Question are:

1. How long should I wait to do hernia repair (to ensure prostate is healed)?

2. Open or laparoscopic repair?

3. Mesh or No Mesh?

4. How long to wait? 6 months or 1 year?

5. What else?

I'm glad to hear your recovery from the HoLEP procedure is going well! Holmium Laser Enucleation of the Prostate (HoLEP) is a minimally invasive procedure to treat an enlarged prostate, and a successful recovery can significantly improve quality of life. When considering an inguinal hernia repair post-HoLEP, here are some insights for the questions you raised, but always consult with your medical professionals for guidance tailored to your specific situation:

  1. Wait Time for Hernia Repair: Typically, it's crucial to ensure that any significant post-operative symptoms from the HoLEP have resolved and that your urinary function is stable. The exact timing can vary, but many surgeons might recommend waiting at least a few months.

  2. Open vs. Laparoscopic Repair: Both open and laparoscopic techniques have their advantages and disadvantages:

    • Open repair is a more traditional approach with a larger incision, which might be suitable for certain types of hernias or patient-specific considerations. It can have a quicker recovery time in some cases.
    • Laparoscopic repair involves several small incisions and the use of a camera to guide the repair. It can be less invasive with a potentially shorter hospital stay and less post-operative pain, but it may require general anesthesia and might have a slightly higher recurrence rate.
  3. Mesh or No Mesh: The use of mesh in hernia repairs can reduce the risk of hernia recurrence. However, there are potential complications, such as chronic pain or mesh migration. Some small or specific types of hernias might be repaired without mesh. The decision to use mesh often depends on the size and location of the hernia, the surgeon's preference, and the patient's specific circumstances.

  4. How Long to Wait? If you're already 4 months post-HoLEP and feeling well, waiting 6 months might be sufficient. However, the exact timing should be discussed with both your urologist (who performed the HoLEP) and the surgeon who will do the hernia repair to ensure all parties agree it's safe to proceed.

  5. What Else?

    • Physical Activity: After your hernia repair, you'll need to follow your surgeon's guidance on resuming physical activity to ensure proper healing and minimize the risk of complications.
    • Chronic Conditions: If you have other health conditions (like diabetes or heart disease), it's essential to ensure they are well-managed before undergoing another surgery.
    • Second Opinion: If ever in doubt, seeking a second opinion can provide additional insights or reassurance regarding the planned procedure.

Ultimately, the decisions regarding the type of hernia repair and timing should be made in collaboration with your healthcare team, considering your overall health, the size and type of hernia, and any potential risks or benefits associated with the different surgical options.
