Lisfrancs - Must screw be removed?

I had surgery for lisfrancs fracture four years ago.  I was told I could leave the screw in, but later was told I should have it removed to keep from possible bone shatters later on.  How likely would that be?  Can the surgery to remove the screw be done using a local rather than full anesthesia?
