As anyone who reads these posts may have gathered, I'm biased. Full disclosure. However, I've had four neck surgeries (two thyroid in 2001, a parathyroid in '03 and another this past March), with fine results. I only know one surgeon but I think I can say that after 13 years he and I know each other pretty well. Doug Fraker does use intraoperative PTH monitoring--and, at least for me (I'm a singer, not professional but very dedicated, and he knows how much it means to me), recurrent laryngeal nerve monitoring. The last surgery involved a pre-op PTH of 48.8 and post-op 6.1, certainly within the normal range (I believe the low is 5.1 and I know the high is 6.5, on Penn's scale). Since then my PTH has been elevated--a few weeks post-op--but steadily descending; in August it was 6.5. My calcium has been low normal, around 9.1. I keep him posted on the numbers, sometimes with a question or two about what they mean and if that's anything I should be concerned about, and yes, he answers. I suppose he has his off days, like everyone else on the planet, but he's very careful, he's as conservative as he can be (once something is removed, after all, it can't be put back) and as aggressive as he has to be. If you're anywhere near Philadelphia or can get here without a great deal of trouble, do yourself a favor and at least go and see him.
