Hi there. I am 21 years old and about nine months ago I had this sharp unpleasant (almost like a push) feeling during ejaculation and my right testicle is about 1.15 times bigger than the left and slightly harder. I went to the doctor right away. He performed a physical exam And said it was nothing to be concerned about and gave me some naproxen to ease the pain and sent me home. During this time I've developed a pain type feeling in my right testicle especially when wearing boxers or just being naked or warm weather. I went back because I was convinced I had cancer. He did another examination and found nothing of concern but to ease my mind he scheduled me for a blood test to check everything all sti s, blood sugar, thyroid, inflammation and blood count. Came back clear and he checked my testicles again and told me everything is normal. I found the pain is almost non existent when I wear supportive underwear. My visits were also spread 2 months apart. So a total of 3 visits and 3 examinations. My testicle did not seem to grow during those months but if it did, it was extremely slow and unoticeble. But it is still a little harder than the other but not rock hard. Should I still be concerned? Thank you.

It's not unusual for there to be some variation in the size of your testicles.  One being slightly bigger is "normal."

The pain you had during ejaculation may be due to a muscle spasm.  There are lots of muscles near the testicles, the cremaster muscles in particular.  Since it seems to lessen when you have support that's where I'd focus.

You might do better with a visit to a urologist.  You've ruled out the major concerns, cancer, STD's, etc.  A urologist specializes in the reproductive tract.
