I am not a physician although this all is very familiar. I have had the same issues. I was eventually diagnosed with ulcerative colitis. I was prescribed Entorcort EC (steroid) and Pentasa 8 per day during flare ups and 4 per day when in remission. I am now taking Humira shots every 2 weeks as I have also been diagnosed with Rheumatoid arthritis. The Humira injections help with both! Good Luck to you. I hope you’re able to get diagnosed and find a medication that will work for you.
Impacted stool / left over stool / incomplete defecation could be the cause. I also have the rumbles and it only stops for a while after a bowel movement. It could also be from a tight pelvic floor that prevents you from defecating properly. If you suspect that visit a pf therapist.
I’ve been using a step stool or those potty step stools and they’ve helped a little. After using it for a while I’ve been okay without using it.

Mine is also completely centred around bowel movements, if I have a laxative there is no problem. Ihad great success with metranidazole which stopped when I stopped taking it, making me think that this is a bacterial issue
