You're going to take this wrong but it is due to masturbation. Trust me, I had the same exact problem. Stop masturbating for a couple of weeks and take it easy. Wear tight fitting underwear that will support your testicles, and take warm baths that only cover you up to your penis. The fact that it comes after sex is the tendency that we as mean have to relive a good sex act repeatedly soon after via self pleasure. Trust me.
I have been having this problem for over 20 years. It just gets worse. I'm in major pain and now my rectum has lumps maybe hemerhoids. I had inguinal hernia surgery and still no trouble found. What is this. Pain shoots in my right testicle right,rectum hip right groin area. I'm freaking,in so much pain I have to hold my breath cause it hurts so bad to breath.
I'm 40. I don't masturbate like I use to. Today I would rather take a 6 pack and go fish rather then have sex. I still have the same problem
Have you checked the symptoms of hpv... only one I know of that has back pain and affects leg as well as genitals. Just a thought.

There is no point in 'scoping out your options' just bite the bullet and get a full sti screening.
There's unfortunately loads of possible origins to such symptoms. The feeling in your urethra could (a bit like its constricted) be anything from enlarged prostate, to uretal stones or even just plain old things like thrush or cystitis (thrush might explain smell).
Testicular pain equally could have many origins - ranging from the big C (god forbid) to some of the things listed above. STI is the most common cause of testicular pain in young men, but if it's not that then you need to see a Urologist.
Finally an important point, you guys haven't mentioned your social circumstances.....but.....guilt of course can manifest itself in many ways including physically. If you're felling guilty for your sexual escapades, and you can find no other cause for the symptoms then a counsellor or psychotherapist is the best course of action!

Guys, we have pruritus ani most likely. Avoid the following foods and anything that contains them. Alcohol, especially beer and wine (drank jack daniels and coke BIG mistake). Milk, nuts, chocolate, tomatoes, carbonated drinks (especially cola), coffee, tea....I've had this problem for over 4 years and my family doctor mistakenly diagnosed me with a fissure on my anus because I complained of rectal itching that was terrible. I went to a rectal surgeon for a consulation and he put scopes in my anus, said everything was fine. Gave me a pamphlet on pruritus ani and prescribed me calmoseptine. I had seen a urologist 3 weeks ago because I thought I had prostatitis. My prostate is fine, it's normal size, feels normal. He even did the smear test in which he massaged my prostate hard and looked at the discharge under a microscope, I didn't have bacterial prostatitis. All I can say is this, if you have an itchy anus, painful testicles, a burning in the urethra, feeling like your bladder is always full, and pressure I would follow that strict diet. Alcohol killed me, I just started following the guidelines of the pamphlet and decided to drink 3 jack and cokes last weekend and my butt is on fire again after feeling fine for the first time in ages along with no pain in my testicles or penis.
This is EXTREMELY important. The rectal surgeon told me to do this. Take fiber supplements! When you have a bowel movement do not wipe yourself with anything, immediately get into the shower and rinse your anus with water if you have a shower head that detaches that's even better. A bidet style toilet seat would be ideal and I'll probably end up purchasing one. DO NOT use soap on your anus at all, it'll mess with the natural oils. I very gently dried my anus with a towel, but today I purchased a hair dryer that has a setting with no heat because I want to dry the area as best as possible without contact. I will never use dry toilet paper again, worst comes to worse I will just wet toilet paper if I'm not at home. When I'm all dry, I put the calmoseptine ointment on my anus which the dr prescribed.

My opinion after battling this for years, is that it's a fungal infection caused by candida. In my case I was diagnosed with thrush 10 yrs ago. It started after first time with girlfriend. Symptoms: rash on penis head and under foreskin, burning/itching urethra, occasional pain in testicles, fish like smell after ejaculation. We were both treated with an antifungal (lotion) but it came back periodically for the next 8 yrs, presumably because the lotion only treated the external symptoms.
2 years ago my symptoms suddenly got much worse with burning around anus as well and then I got a dose of gastritis/reflux, in my opinion all related to the fungus. Blood tests have all been negative and doctors had no clue.
The reason I believe this is fungal is because fungus feed on sugars and when you ejaculate your effectively feeding the bacteria and fungus in your urethra. If you eat a lot of sweet food or bread, pasta etc even your urine will contain sugars your body doesn't consume.
As well as all the genital related ailments I had, I also had the following symptoms which got worse in the past 2 years:
- fungal infection in toe nails
- athletes foot
- skin infection in armpit with red pustules (became very nasty)
- 'pick axe' headaches. These were not your usual headaches, but pain in the nerves around the top of the skull, not in the brain. Excruciating pain but impossible to treat because they would not last more than a minute.
- nerve pain all over body
- numbness in finger tips
How to treat yourself: I have recovered from all of these symptoms very slowly by cutting out sugar, alcohol, gluten and reducing sexual activity. I've essentially followed a paleo diet. It's not easy and the hardest parts for me are the alcohol and the sex but I have no choice. The only alternative is fluconazole but this can cause havoc with your stomach and given that I had gastritis I haven't gone down that route yet.
This may not apply for all, but I think this is the cause. Try the diet for a month & see how you go. Beware though, after a week or 2 you will get candida die off symptoms which don't last long but will be as bad as or worse than what you feel when ill. When this happened to me the skin around the tip of the penis dried up and became flaky. Each time I pushed the diet harder the same thing happened but the area affected shrank, as if the fungus had less to feed on and was shrinking. Good luck to all!
