I have seen urologists before and neither they nor I have ever noticed this before but yesterday as I was checking my ball sack I noticed I now have three testicles. There is a little pain while checking them but not much. What could cause this to happen?

You don't just suddenly "grow" a third testicle.
See a urologist and get it checked out. It can indicate a cyst or even a tumor.
