Itchy acne which are bleeding and surrounding skin has become like cracked skin

Hey so since almost a month

i have a kind of acne..it is itchy and i cant sleep at night..so i scratch it and it bleeds almost every day...the surrounding skin has become like cracked skin...Should i consult a doctor ?? or should i just put some cream on it,,,??
