Problem with Bulky adenomyotic uterus

Patient history : Name : Mumtaz Begum; Female; 38 Years Old; Single (No Children).

Problem as reported by Hospital
23/01/2010 - EXAMINATION : USG ABDOMEN & PELVIS
IMPRESSION : Bulky uterus with multiple intra mural fibroids & adenomyotic uterus.
The uterus is anteverted, bulky and measures 113 x 64 x 70 mms.
Endometrium measures 8 mms displaced posteriorly because of fibroids.
Multiple variable size intra mural fibroids in anterior myometrium size of approximate 13 x 13 cm with hypoechoic echotexture.
Both the ovaries are normal in size & shape. No focal or solid lesion is seen.
No adnexal abnormality is seen. No free fluid in the pouch of Douglas.

27/01/2010 - EXAMINATION : CT Scan Adbomen + Pelvis (P + C)
IMPRESSION : Small mesentric adenopathy is noted.
Bulky adenomyotic uterus - Uterus is bulky adenomyotic

Only a few questions :
1) Is this a serious problem or can this lead to serious problems in the future; 2) Does this require any MAJOR corrective operation; 3) Is it related to problem of deep pain every month during mensuration; 4) Is it any way related to inability to concieve.
