Torn foreskin tip and swelling

Hi there, I'm 20 years old whose had 28 sexual partners. Never had an STI. Recently over the past 3 months if I am constant in sex and masturbation my forskin tip, not my banjo, (sorry don't no medical term) is ripped, and swollen to the point I can't touch it. I've used steroid cream on it in the past but I'm fed up of it returning as its preventing me and my girlfriend to have a healthy relationship. Forskin looks like it's been torn, 5-6 tiny cuts going down my penis, forskin in inflamed and tender to touch
