I have had this same issue over the past few months. I finally went to a Colon and Rectal doctor and he said it is very common - it is Pruritus Ani. He had me get Calmoseptine ointment and said i would notice significant relieve within three days but it would take a little longer for all the symptoms to go away (red sore areas, whitening of the skin, scratchy areas, etc) I have noticed a significant relief since i went to doctor two days ago.

I am a male and have dealt with this problem for over 15 years and for most of that time considered all the horrible possiblities mentioned in this thread. Finally, after years of going insane and with no doctor having a clue, I discovered that the problem is was a YEAST INFECTION, likely brought on by a period where I took a large amount of antibiotics. The way to tell for yourself is take a Diflucan pill and see if that lessens the symptoms. You should notice a difference the next day, if yeast is the problem.
The problem with a yeast infection, though, is once you have one like his, it will likely flair up AGAIN AND AGAIN, even after you take the Diflucan, for the rest of your life -- if and when you eat sugar. This means, if you want to be itch free, you CANNOT EVER eat ANY FRUIT OR ANYTHING ELSE THAT CONTAINS ANY KIND OF SUGAR. Try using Stevia (KAL brand is the best) as a substitute, since it is sweet but contains no sugar at all. If by chance you do eat sugar, and you will know when you did by the itch you get, yogurt and strict sugar/fruit avoidance will alleviate the itching over time. Remember, sugar is in virtually every processed food, so read the label carefully, looking to avoid things, like "honey" "fruit-juice-sweetened" "corn syrup solids," "brown rich syrup", "Maltodextrin," Dextrose, (even Nutra Sweet and Splenda will trigger it).
I have not found that unsweetened white flour products have any affect on it, btw. Good luck everyone!
