Ok, so not very common, but was diagnosed with a Urachal Remnant.
Google it, sounds viable.
Can flare up due to infection or affect nerves in general.

Hi, I'm a 21 years old male and I also have an innie belly button. I have been suffering from what I feel is the exact same pain as you guys for about 15 years already.
My pain also goes from my belly button to my penis. I've been to plenty of doctors and they have done plenty of medical tests already but they did not find anything wrong with me and said the pain was in my head. Because of this I have stopped doing any physical exercise for 5 years as it would often trigger this pain after I was done when I would go from a sitting position to a standing position. However I still get this pain at least 3 to 4 times a month without ever exercising. When it happens I can't stand up because of how much it hurts. It hurts a lot also when I pee. I have to sit down or lay in the fetal position for about 6 hours each time when this happens for it to get better.
I have read all of these posts so far but haven't really understood or figured out what was the cause of the pain for you guys and I also would love to know what specific doctors I should go to. Thanks in advance for your help!

For me, I think the answer was 3), which is the hardest to diagnose, in my opinion. Here are my thoughts on each:
1) Umbilical hernia - this will usually show up on a CT scan. I had a scan done, it didn't show up, but I ended up having umbilical hernia surgery anyways because I was so desperate to try anything to fix it (everyone has a small umbilical hernia and a surgeon can always sew it up - I had this done but it didn't solve my issue).
2) Urachal remnant - I think an ultrasound can help see this, seems like this was the issue for many here, and can be helped by surgery.
3) Irritated nerve - It took me a long time but I finally figured out this must be what I had. I have always had a very sensitive belly button (If you poke it straight on I get an extreme discomfort shooting from my belly button to my testicles. I've had it as far back as I can remember I'm 39 now. I have an innie).
Every once in awhile I'd get the bad shooting pain but it'd tend to go away after a week or so. However I had a vasectomy which, my theory is, irritated that particular nerve such that the shooting/awful pain came back. I took gabapentin which helped it subside, but it didn't completely go away so I had the hernia surery because nobody could really figure out the issue (now I'd say the surgery was probably not necessary). The surgery brought BACK the horrible shooting (the area again was probably irritated like it was during my vasectomy), so I healed up, and went back on gabapentin. I was up to 1800 MG of Gabapentin a day. Gabapentin seems to have helped, and I have now decreased my usage of gabapentin and am feeling okay. I'm not quite back to where I was pre-vasectomy, but I'm in pretty good shape.
I'd recommend, if you've ruled out umbilical hernia or urachal remnant to try to get an Rx for gabapentin and see if it works. The next step if Gabapentin doesn't work is likely to do steroid injections into the lower abdomen (a surgeon can try to find the particular never and inject steroids). I was actually scheduled to do this but was feeling good enough that I cancelled it. However if the shooting pain comes back, I will be scheduling injections for sure, as I don't want to depend on gabapentin forever.
Really hope my story helps someone - I've been dealing with this for 3 months now and I understand it's absolutely crippling and frustrating.

It may not be the same thing in others but it could be a specific food type or maybe you need to get tested for ceoliac disease.
Ceoliac causes this type of pain as it flattens the intestine lining which becomes painful.
It makes sense for it to be food related in someway as tests show negative for anything else. If you had a blood test they probably wood not test for ceoliac disease.
Next time you get pain try an think of what you have eaten that week like lots of bread,cakes or like me onion (not related to ceoliac) that may be a trigger for the pain.
