Sounds like you are doing well too. I think yiu are the first to mention Vanderbilt. Sounds like it went well, Next month will be 2 years since my HoLep and I'm schedule to see my surgeon Drr. Das in Philly on 12/23 for my annual checkup with him.

Thank you Barry for sharing such critical informaitonon HoLEP procedure. I am in the midst of a Green Light laser surgery for BPH. I'm trying to educate myself on the best options. Your insight has been great although I'm still trying to undestand your overall fate meaning you will always have to urinate often or this is just part of the overall healing process?

Tony: I'd like to suggest (as I did a few posts earlier" that you search for, read and follow the other thread here on Steadyheath. It's called " It is called"Experiences with Holmium Laser Enucleation of the Prostate, HoLEP, Laser Surgery" . Check it out. There are more folks who had the surgery there and are sharing experiehnces. I only recently found this thread about best doctors. The other is "experiences". See ya over there.

Have you consulted with Dr. Dora and if so with what results? I am preparing to make an appointment to discuss the procedure after being referred by my urologist.

Here is HoLap and HoLep, both use the Holmium laser. HoLap is an ablation surgery
and HoLep is an encleation surgery. They both use the same laser, but the HoLep also uses a morcellator to cut and chop the tissue fron the prostate. On the internet they
talk about the HoLap being much easier. And going home the same day, with less pain
and bleeding. I'm at a point where I', tying to determine which is best for me.
Has anyone out had the HoLap done?

I have not read anything about BPH treatment. There seems to be so many differant possible treatments usung Lasers, that it is hard to know which
is best.

I am not an MD, but I have prostate cancer and a very large prostate and I have been researching laser surgery in quite some detail. As I understand it, HoLAP (ablation) uses a laser to cut a channel through a portion of the prostate material (adenoma) that reduces pressure on the urethra, thus allowing more-or-less normal urination. In this procedure only a portion of the prostate contents (adenoma) is removed. However, I was told by more than one urologist that the adenoma can grow back and re-create the pressure on the urethra, which could necessitate another round of HoLAP or other surgery at some future date to remove the new adenoma tissue.

In contrast, HoLEP (enucleation) involves using a laser to separate the internal content of the prostate (adenoma) from the shell of the prostate, and then grinding up the pieces of adenoma and sucking them out. In this case all or virtually all of the prostate content (adenoma) is removed, so in theory there is little or nothing left to grow back and the incidence of repeat surgery is lower than with HoLAP.

Alert: Whatever prostate removal treatment you favor, be sure to discuss with your urologist/surgeon the size of your prostate. Estimates of my prostate size range from 130 to 200 grams, which is pretty big and my HoLEP surgeon says this may require cutting an opening in my urethra behind the testicles to ensure that his HoLEP tools can reach all the way to the far edges of the prostate (ouch!). Cutting this opening will likely increase the amount of time in surgery, the amount of time with a catheter after surgery (1 week vs 1-2 days), the potential for bleeding, and the amount of hospital time after surgery (several days vs 1-2 days).

Also, HoLEP surgery is very complex and has a steep learning curve, so be sure you choose a urologist/surgeon who has done at least 50, preferably more, HoLEP surgeries (it's OK to ask, he or she will tell you how many; if he/she won't tell you, choose another doc). This is even more important in the case of large prostates (greater than 100 g). HoLEP apparently does the job even for large prostates, it is just more complicated and you definitely want a surgeon who has done HoLEP on many large prostates.

Again, I am not an MD, so be sure to discuss with your urologist/surgeon these topics and any other questions or concerns you may have.

Which surgeon have you consulted? Have you tried Dr. Lingeman in Indianapolis? Can a HOLEP be done when someone is having prostate cancer surgery? I had a partial urethrotomy done when I had HOLEP 11 months ago, but, the incision was not near the testicles. It was an incision at the opening of my urethra. I only had a catheter in for about 10 hours. Is your procedure more intensive because of having cancer?

I am scheduled to have Dr. Dusing do my Holep surgery in the next few weeks. How has your husband fared since the surgery?---any significant side effects or regrets?

My husband just had the HoLep procedure done by Dr. Humph ties at The Mayo Clinic in Phoenix AZ. Very pleased with him and the staff at Mayo Clinic. It is an amazing facility and Dr. Humphries is very experienced at HoLep. If anyone is on the west coast, this is the place to go.
