Same old, same old here... CT scan, cystoscopy, etc ALL negative. Urologist was telling me having blood/clots in semen and blood is normal and usually benign. My general doc told me the same thing. I told them both I don't want to hear it. I want it FIXED!

As a last resort, (this is going to sound crazy to most of you as it did myself) the urologist sent me to physical therapy for pelvic floor dysfunction. I said... are you high?!?!? However, he explained the blood is most likely from inflammation of the urethra, prostate or my spine being out of alignment. I've been doing the pelvic floor muscle stretches/exercises on top of many other back stretches and additional exercise. Haven't had blood or clots in urine after sex for 3 months. I know that's not long but I just feel different and truly seems like its solved. I have three more months of therapy and then heading back to the doc.

I'll update if anything changes but give this a shot. Basically standard back/spine exercises and stretches along with pelvic floor muscle exercises. Most of the stuff I was shown in therapy can be found on Google/You tube videos. Hardest part is figuring out how to contract the pelvic floor muscle correctly.

I think this may explain why most people dealing with this issue experience periods of remission then periods of recurrence because the underlying issue (in probably a majority of those that have done endless tests which all came up negative) of the pelvic floor muscles being strained have not been addressed. I've just recently experience the same blood in urine and semen after and during sex a couple of weeks ago after a month of feeling discomfort when urinating and excessive urination along with pressure/heaviness in my lower abdomen. I am abstaining from sex and start the pelvic floor therapy while continuing to rule out the serious stuff (blood and urine tests came back clean, scan next) and will update my progress. My gut says STRESS is the underlying culprit which in turn is inflaming the pelvic region resulting in the inflammation of the bladder, urethra, etc causing blood vessels to bust when the blood flow increases due to being erect and doing what men do

I have had this same exact problem off an on since 2004. Im 37 yrs old and have had this since 23. It usually happens once a year to year and a half and lasts for about 3-4 weeks. I’m married and have been for 10 years. I have 1 healthy kid, so this doesn’t affect whether or not you can procreate.
A few things I’ve noticed:
- it happens from rough sex. For me it has happened when I’m entering from behind. So I avoid that. I think it has to do with the angle and causes a rupture of a blood vessel in penis
- it’s important to lay off any activity for 2-3 weeks. Don’t get aroused. Avoid wanking and sex. Your body needs to heal the broken blood vessel. Not easy but if you mess around you’ve got to start the clock over
- avoid alcohol and inflammatory foods like sugar during the healing time. The cleaner you eat the quicker it’ll heal.
- take 1000mg of vitamin c and a b vitamin complex. B vitamins and vitamin c help to heal blood vessels
- drink nettle tea. Research nettle. It works and it helps me. I buy loose leaf organic and brew it myself. Nettle is high in Vitamin K (which another member mentioned) and it also decreases inflammation in the prostate.

I’ve had all the tests you guys have had. They’ve always come back negative. I have learned what I need to do to heal the ruptured vessel. I’m curious if it’s the same vessel Everytime? The key is to lay off sex and let it heal. Take the supplements i mentioned above and I’m usually healed up in 2-3 weeks and good for another 11 months or so.

Hey all,

So I was someone who had this problem for nearly two months. After hours and hours and hours of researching this and literally losing my mind over this, I've been able to come to a few conclusions. I hope this is the post to end mostly all questions and help you all.

It may be a sign of hematospermia (which is harmless and self-limiting) if it happens only once or twice every so often, but if this is a constant or recurring thing, that takes weeks or months to go away, this is something that needs proper medical attention.

In terms of possible diagnoses:
For most people on here, it's most likely prostatitis, an inflamed or infected prostate, which could happen from unprotected sex or sexual behaviors, bacterium that can infect the urinary tract, etc. Other symptoms of this will include painful ejaculation, burning in urethra, pelvic pain, waves of pain in the penis, scrotum, or pelvis/lower back, frequent urination, blood in the ejaculation, etc. Antibiotics will be your first shot at getting rid of this. Most urinalysis tests won't find this, it will require a smart doctor or someone to check your prostate efficiently by running the right tests, checking your PSA (bloodwork), or trying to examine the bacterium makeup in your semen by doing a prostate massage. A cystoscopy won't find this either usually unless your prostate is enlarged (BPH). If antibiotics don't work, then it's not bacterial/infection based--it could then be a prostatitis that's caused by your pelvic floor muscles not working properly. Doing exercises to work on this will help you, research CP/CPPS (chronic prostatitis/ chronic pelvic pain syndrome) for more details.

If your prostate and pelvic floor is 100% clean and clear and fine, then it could be vasculitis or a gout issue, in which your blood vessels can break or pop easily. This could happen due to an autoimmune reaction. This will require bloodwork to check for uric acid levels if it's gout-based, or if it's vasculitis, you will need a doctor to confirm this through the right tests.

If neither of those work, then it could be a urethral hemangioma or a polyp. This will be confirmed by a cystoscopy and studying your erections if it's something more hidden. A doppler study is the test you could run to figure out if it's a bloodflow/vessel related issue. An MRI may also confirm this too. Again, insurance may be an issue for this so please seek the right help if these tests are too expensive. Not everyone has the right access to this.

For more details, google or research the term 'Post-coital hematuria' which seems to be the official terminology by medical research and urologists. It's a very rare symptom but it can be traced back to usually prostatitis or even a bladder infection, possibly gout/uric acid issues, possibly vasculitis, or even a urethral hemangioma/polyp/vessel malformation. Talk to your doctor about prostatitis if your symptoms match.

Research prostate health, diet, and supplements if you feel it will help. Do pelvic floor exercises and try not to sit for too long.

I’m adding this in the Hopes that it’ll help someone figure things out. Your situation might not exactly be the same as mine, but blood in urine is never good; even if it’s only when you have sex. Everything you’re all explaining here I’ve been experiencing since 2013. It never happens on a normal basis, only after sex, or ejaculation…. Worst case scenario (when it was really bad) from simply being aroused. I’ve had those embarrassing moments when you’re with a lover in the heat of the moment and it happens. I’ve had it when I was wearing white shorts; bled all over my favorite white shorts. It gives you a little bit of sexual anxiety. Lot of sexual anxiety, sometimes you’re scared to evacuate because you’re afraid of what might come out. You’re always on edge in intimate moments. It’s been 8 years dealing with this. I’d done all the tests, mri, mri with dye, cystoscopy, and I’d given up. Only thing that would help was ciproflaxon, or I’d stock up on Doxicyclin for when the episodes would come. And they WOULD come and go, whenever.

Check with your urologist, go through the steps, even if they don’t have the answers, keep monitoring it. When we did all this 8 years ago, there were no answers for me. 8 years later, we did it again, and they found a giant benign legion (that can be sometimes cancerous) growing in my bladder. “Cystitis Glandularis” is a rare (VERY Rare) form of legion that grows if in bladder (8 years and it was now big enough to catch). Google it, they produce a lot of mucus, it thins out the lining of the bladder, Which might explain why when blood rushes down there, the mucus probably soaks it up, (though the thin walls) and it comes out in these thick mucousy clots. A uriniary tract infection is usually a symptom of CG. Again, I’m not a doctor, but it always felt like blood was going down the wrong pipe. This can happen at any age.

After 8 years, I’m not jumping the gun to say this is the answer and I’ve been cured, we’ll see in 6 months. But they did go in and cut the damn thing out, and it’s been better; like night an day the pst month. I’ll know for sure in 6 months or so. After 8 years it’s hard to just accept that I’ve been cured…. But I wanted to put this out there, because a penis malfunction with no answers is SCARYAF. Check in with your doctors regularly, and watch it. I was even diagnosed once with kidney stones. If this is possibly what’s happening with you, if it goes unchecked, it can become cancerous. I KNOW WHAT THIS PROBLEM IS AND HOW TO CURE IT.

As many of you here, I’ve been dealing with this problem for a long time—about 6 years. The symptoms are exactly the same as you all describe: blood in semen, and then pure blood with clots comes out of the penis when you urinate. Sometimes clots are so big that they block urine and you can’t pee. Only happens after sex or long and strong erection.

I visited all the best urologists in my city. Gone through numerous ultrasound scans and even tomography of pelvic organs. Nothing showed the problem. None of the doctors helped. They treated me for prostatitis, vesiculitis, and even phlebeurysm of pelvic organs. I did lots of different injections, took various antibiotics, did urethral instillations. Nothing helped and gave a sustained result.

And finally, my urologist who was treating me at that time gave me contacts of professor of urology in the neighbor city. The professor specializes on urethroscopy.

So, I visited him, he put a camera in my urethra and noticed problematic areas there. A normal urethra has paraurethral glands all the way from a penis to a bladder. And some of these glands didn’t looked good in my urethra. The doc took a tissue sample from these glands and sent to analyze.

And they found a PARASITE in it. It is called Schistosoma. It penetrates urethra when you swim in ponds, rivers, lakes where this parasite lives. It normally lives in South America, Africa and a couple of other countries including the US. When you pee in the water, Schistosoma is attracted by your urine and the risk rises.

So, what Schistosoma does when it gets to your urethra: it stays in those paraurethral glands and starts reproducing. These little bastards literally bite through tissue and make a hole that connects a paraurethral gland (and it means urethra itself) with spongy body of your penis where there is a massive blood flow when the penis is erected (this explains why blood comes out only after strong erection). So, when you have erection, sex, or whatever, the damaged tissue can’t stand the blood pressure in penis, it tears, and blood from spongy body of the penis goes out to the urethra through paraurethral glands.

This is the explanation the professor urologist gave me. Simple as that, guys.

So, how to cure it.

The urologist simply flushes paraurethral glands with the right medicine to kill the parasite. The procedure is done visually through a urethroscope, of course. You will need to do this procedure multiple times with a 3-4 week break after each procedure. That’s what I’m doing now.

What you need to do is to find a good urologist that has a urethroscope and knows how to use it. I mean a GOOD urologist. Tell him to take samples from paraurethral glands and analyze them for Schistosoma.

Yes, you can’t go without urethroscopy but it’s not that scary and painful as you might think.

Hey Everyone - Below is my story and I think it can help a lot of people with this issue and hopefully start a discussion to help permanently heal many of us if we share anything that worked for us.

BTW - Been following this post for 3 Years.

Back in August 2018 (I was 41) I had intense sex with my wife (After drinking in Excess) and ejaculated Blood in my Semen followed up after Urinating Blood and 2 Clots after.  A backstory to this was I was taking Over the Counter Erection Pills as I had a minor addiction of the type of erection they could give me even if I drank a bit too much Alcohol one night or was in a hot tub prior to sex.  Taking these Pills made it easy to get a very hard erection.  Now I get why doctors say do not take this stuff.

Went to the Doctor and had the following Test Done:

Blood Test, Urine Test, Semen Test, Regular Ultrasound, Rectal Ultrasound, and MRI done.  Doctor could not find any damage, bacteria, or viruses.

During this time I continue to have sex with my wife and I would CONTINUE to have Blood in my Semen when having sex followed by Urinating blood and 2 Clots after.  If I masturbated there would be NO Blood in my Semen or urinating clots.

This baffled my Doctor.  Doctor did not want to prescribe my Anti-Biotics as my testing said I was fine.  After some quick personal research I came to a quick conclusion I had Non-Bacterial Prostatitis (which I did not have).  So I tried to treat my issue naturally with that assumption.  Tried Colloidal Silver and Cranberry Juice thinking this could kill any weird bacteria.

Crazy thing that happened I had sex with my wife and there was very little blood in my semen and when I urinated after I urinated some blood but no clots!!!!  I went back to my doctor and he prescribed me Dioxycyline the doctor thinking it MUST be Bacterial Infection.  In addition, because Cranberry Juice helped, I took Cranberry Pill Supplements.  Within 2 Days there was NO Blood in my Semen and no Urinating Blood with 2 Clots after.

I was cured until December 2020.  Another backstory to this I continue to take small amounts of Viagra from the time I was cured to December 2020 (Stupid Me but never had a problem).  December 2020 having aggression sex (Also, Drinking Prior to Sex)  and I had Blood in Semen and Urinate 2 Clots.  Went back on Dioxycyline and Cranberry Supplements.  Cured the issue.  However, took Viagra and happened again within 4 Weeks.

So stopped Sex Pills Completely and just took the Cranberry Supplements and Cured but NOW I am afraid to have more aggressive sex LOL.  I probably should be happy it is not happening but I am hoping I can totally cure this issue.

So what I come to conclusion is many of us have a Rupture Blood Vessel on or very near the Prostate / Ejactulatory Ducts.  These Blood Vessels are weak and rupture during sex or masturbation, depending on how severe the rupture is.  I think my Rupture is not too bad as I do not have issues peeing out the Blood Clot, they Pee out pretty easily.  Taking any Sex Pills makes it worst as it cause way more blood to flow down your penis which in conclusion makes it easy to Rupture the Weak Blood Vessel.

I also think drinking causes the Heal Blood Vessel to Rupture more easily as Alcohol thins the blood.  Side note, I realized every time I ingested Craft Style Beer which is not often it occurred.  I think it irritates the weak Blood Vessels in the Urethra / Prostate / Ejactulatory Duct Area.

So basically I have come to the following treatment with what I have found works for me and also what I have read on this Post and other Forums:

Dioxycyline - This is not for the Antibiotics reason, this has a Anti-Inflammatory which helps in the healing process.  I have not taken this recently but I think it speed up the healing process.  I am thinking if someone could recommend another Anti Inflammatory supplement.  Something more powerful.

Cranberry Supplements - Not sure why this works but it must help heal the rupture blood vessel

Turmeric Supplements - Read this on another Site helps similar to Cranberry

Vitamin K with D - Helps to Heal the Rupture Blood Vessel (Will a Higher Dose help????)

Vitamin E - Helps with Healing (Will a Higher Dose Help????)

Vitamin C - I take a lot of this hoping it helps with Healing

Prostate Shrinking Supplements - This helps the Blood Vessels not Rupture as your prostate is smaller.  Like taking Saw Palmetto, only thing I find on Saw Palmetto its harder to get an Erection.  But that could just a side effect for me.

Drink Less Alcohol before Sex

If anyone else have any supplement Ideas?

I am also thinking on Over dosing on the Vitamins but maybe people have thoughts on this.  I know sometimes its better to take more Supplement then the bottle directs. 

I think to help fix this problem long term is to take the right Supplements and right amounts to help the body heal the Rupture Blood Vessels.

I am going to Try a higher dosage of Vitamin E - Called E Annatto Tocotrienols Deltagold - Amazon Reviews says this is the best type of Vitamin E and from other Research Online.

One thing I read on a few other forums, and from my research, and from basically my Urologist.  If any of you are desperate and want to fix this.  Take Prescription Finistride.  This will shrink your prostate and it has other attributes which helps you not bleed in you penis area.  Only thing that sucks is it make you have Erectile Dysfunction.  Also, could take 6-8 Weeks to kick in and my take that time to get off of it.  Also, I have read it can have other crazy side effects.

I am ready to go that far yet as I have been managing taken Supplement.  But maybe we can find other Supplements that can help.

I don't drink any alcohol but still have the problem. Maybe I need to lay off garlic because that can make you bleed easily.
