I've recently been diagnosed with a left sided varicocele. I had an accident last week where the blood in my penis was forced backwards pretty hard while fully erect and I felt a sharp pain. I now notice that the bottom of my shaft when erect is swollen on the right side and that my dorsal vein that runs along the top of my penis does not fill like it used to. It seems to be almost deflated.

This is a little awkward to answer.
First, you indicate that you had an "accident with the penis" but it's not clear what would have forced the blood back in the penis with such force that it might rupture the vessel. The source of this force, whether through sex, masturbation or abuse, would shed a lot of light on what happened.
You need to be examined by a medical person. It sounds like you may have popped a blood vessel but I'm not qualified, nor able, to make that assessment.
You say you are observing that the dorsal vein does not fill as it used to, and you have some swelling and felt a sharp pain.
It all ads up to a trip to see the doctor. I would urge you to go sooner, rather than later, so that if some corrective surgery might be needed, that it can be done before the body attempts to heal itself. And there might be more complications. Are you bruised? Discolored? Bleeding?
As for having a varicocele, I don't see how that would affect any of this.

From your description of a swollen shaft and the penis not filling like it used to it sounds as if you've fractured your penis. Basicallly, you've "popped" one of the chambers in your penis that causes erections.
It happens.
You do need to see a doctor, ASAP. It is a medical emergency and there are long term consequences, including problems with erections if it is not treated early.
