I am two years post holep. Have occasional slight leakage when bladder is full and I am active.surgery was very successful even though still get up one or two times per night and have irregular urination patterns. Since the surgery I always feel like I can push urine out any time and must empty bladder before sex or urine can come out during climax. Very minor inconveniences as to life before surgery.
Two months ago developed some urethral discomfort for a few days and have since developed reduced flow. Still urinate well, usually 8-12 ounces per urination but flow is diminished with the feeling of some urethral pressure.
I believe I developed a urethral stricture.
Has anyone had a similar experience.

As I posted a few weeks ago I developed a urethral stricture only about 2 months after surgery although I have read it can develop up to 2 years after. My flow rate gradually reduced until it was as low as 3ml/sec and I experienced the urethral pressure that you say you are feeling. I had a cystoscopy and the surgeon could not get the camera past the stricture. I had surgery just 4 days later, on 23 September 2015, and the stricture was opened up by cutting along its length (only about 2 cm long). I was released next day with a catheter which blocked that evening unfortunately which required an emergency home visit by the community nurse. In consultation with my surgeon she decided not to insert a new catheter. Within 24 hours my flow rate was immensely improved and has remained so and is generally an average of 17ml/sec with a maximum of 600ml total urination, although it is normally around 250-300ml each urination during the day. I can often last all night without the need to urinate and it is only ever once per night if not.
So, up to now that operation has been a complete success and the surgeon said there was more than a 50% probability it won't reform. If it does I will probably need a more radical procedure involving cutting out the damaged part of the urethra and joining the two ends together. This has a 95% success rate but few surgeons in UK carry this out.
In my case the stricture was near the top of the urethra just downstream of the sphincter and this type enjoys the best success rate (although still only 50% plus) whereas strictures in the penis itself have lower success rates apparently.
I think you certainly need to ask for a cystoscopy to see exactly what and where the problem is.

I am just thrilled with the results a week out from surgery. No blood and almost no leakage.

Thanks, Steve. Early days, but this looks to be much superior to all the TURPs I watched over the years of my anesthesia practice. No bleeding, no transfusions, no impotence, out of the hospital the next day.

My Mother is downsizing. Just recently,she gave me a collection of OLD newspapers relating to some of my early life events. In one of these papers, I noticed the name of a friend of mine in the hospital news. {They dont do this anymore} Years ago,this friend told me ,he had chronic Kidney stones requiring surgery to remove them. The post in the paper was the last time he had surgery. He had been cut on so many times he could no longer have surgery. He told me ,The next time , he would die, and he did. He was 25 years old. I was transferred from my bed to the operating table in a prep room at IU Methodist Hospital. I could have touch the equipment Dr Lingeman invented that would have save my friends life. It just wasn`t in time for him. Dr Lingeman however did changed my life with the Holep he performed on me. Moments like these make you stop, and Thank God for his Blessings and Mercy. Drs are not God, but they are as God . They do things its seems only God can do. They do the cutting and God does the healing. Thank You Dr Lingeman and all your staff for your sacrifice and dedication to excellence . I saw the Lord God and he Looked a lot like You. Just Me

May have spoken too soon. On the seventh post op night, I got up a few times to void. Had a sensation of needing to have a bowel movement, but didn't. When I woke up on Thursday, I had soreness at the rectum. Reached down to find a grape sized thrombosed external hemorrhoid. In the early days, I had to strain to have a BM due to the pain meds. I am sure it was due to the earlier straining. Two days out it is sore and I am treating it with Meta Mucil and cleaning it with witch hazel wipes. Hoping that it will go away after a couple of weeks. I have no blood in my urine, and am fully continent. But for the hemorrhoid, I could convince myself that I had had nothing done.

Just back from my annual checkup with Dr. Das here in Philly at Jefferson University Hospital. Did my flow test, had the ultrasound and got my digit exam (was told I could do every two years but it's not painful so I had it). Everything was fine,.. PSA test will be done with my annual physical in March. If I drink a LOT I may get up twice a night (rare) but otherwise, just once. And no pills. Just past 3 years since surgery.

Glad for you Steve. I'm 6 months post op, and also just get up once per night and rarely twice. No pills, 0.8 psa and don't have to stop on the highway every hour like pre-op.

Bruce - I am a doc myself and had the surgery done in Tucson, AZ, by Dr Joel Funk. he is at U of AZ/Banner Med Ctr. He is awesome! Go, as in yesterday! I flew in from SC so you know Zi did my research. Go and get it done. Cath was out in 17 hrs.

I just posted a message and it didn't seem to go to this forum. I am just 4 weeks away from HOLEP surgery with Dr. Lingeman. After being referred to him by my urologist, I think I have made a wonderful choice in a physician. If I can figure out how to get my posts in the right forum, I will write more as I progress in this adventure.

I look forward to hearing how your surgery and recovery goes. My father (now 82) had a HoLEP with Dr. Lingeman many years ago (200+ grams removed) and has recommended the procedure to anyone who would listen since then. Now after 7 years of trying more conservative treatment methods with Hytrin, Avodart, and Rapaflo, today I scheduled my HoLEP surgery with Dr. Lingeman. At the time of my last biopsy in '09, ultrasound measured my prostate at 119g. I've finally had enough with the symptoms and am ready to travel from Texas to Indy for surgery in May.

I've read (in medical journals) that while HoLEP dramatically improves BPH symptoms (e.g., flow, nocturia, urge, etc.), retrograde ejaculation is almost a certainty. I'm curious what the experiences of those who have actually had the procedure are with regard to retrograde ejaculation.
