ABC, I have had banding done and it was temporary. The hemorrhoids returned a year later. The TDH procedure is supposed to be permanent. Your Dr. may be skipping the banding to save you the trouble of a second surgery.

I have been suffering from hemorrhoids for probably 40+ years. I am 67. Deciding to finally do something about them, I went to my Gastroenterologist. I just finished my 4th rubber band ligation 2 days ago. The homorrhoids seem to keep forming! My doctor suggested doing some research on Transanal Hemorrhoidal Dearterialization (THD). Coming to this site for the first time, it appears that THD has been more positive than negative, so I made an appointment to be evaluated for that procedure next week.
Yesterday morning I awoke and the external hemorrhoids were the worst ever. It was a struggle just to walk. Wiping after a BM was quite painful. My gastro doc had told me to quit using wet wipes and quit using Anucort but instead told me to use Balneol lotion for cleaning and then prescribed Hydrocortisone Acetate 2.5% Pramoxine HCL 1% cream to be applied daily as needed. These things did nothing to allieviate the pain, blood or itching.
I remembered something that had 1 worked for me in the past. This is the important thing i want to pass on to everyone. Yesterday morning about 10AM I took 2 Loperamide Hydrochloridl Tablets 2 MG which are meant to be used for diahrrea but work wonderfully for my current condition. In about 1 hour my pain started to subside. Yesterday afternoon and evening and night went without any pain or itching. The blood lessened to almost nothing. I could wipe myself without pain. The next day (today) until 4PM i had no pain or itching.
I realize that you cannot continue to take anti-diahrrea medication for too long without stopping yourself up. I have been able to have normal BM's starting last night and into today. I highly recommend this for anyone who is in constant pain and itching due to internal and/or external hemorrhoids. If my itching persists or worstens, i will take one pill tomorrow morning - in other words waiting a day inbetween doses.
One other thing for those of you who 'leak' blood or stool. I have started using thin Maxie Pads bought at Dollar General. They are easy to 'install' and work great.
I will reply again if I end up having that THD procedure.
Wow...thanks for that. I'm getting it next tues and I'm hearing so many horror stories. That made things a lot easier for me.

I can't tell you how happy I was to come across your post. I recently had a doctor's exam as I have been bleeding with every bowel movement. The doctor suggested I have the THD procedure done as this would be the best option for me. I'm currently waiting for clearance from my Cardiologist. Thanks again for putting me at ease by sharing your experience.

Very glad I had THD, even though it was NOT the 48-hour recovery predicted by my doctor and cited on the industry web sites. More like two to three weeks. As they say, everyone is different, and in my case I had "tenesmus" for two to three weeks -- the constant feeling of needing to "go." The nerves in that area, having been disturbed, were basically fooling me into believing that I had to "go" all the time. Also, in my case, bowel movements were painful for the first 10 to 14 days.
I am a long-haul international pilot, so I couldn't go to work in marginal condition. When I complained of my situation to my doctor, he predicted all would be well after three weeks. That turned out to be true.
I remain VERY happy that I had the THD, but I really wish the industry and the doctors would be more up-front about the highly variable individual experiences, instead of quoting the best-case outcome to everyone.
P.S. For temporary relief of hemorrhoid pain/itching, I recommend just a dab of Desitin Cream -- baby diaper rash ointment. It's a simple white zinc-oxide ointment in a tube, inexpensive, and very effective. Glad I had it when I needed it. Glad I don't need it any more.
