ABC, I have had banding done and it was temporary. The hemorrhoids returned a year later. The TDH procedure is supposed to be permanent. Your Dr. may be skipping the banding to save you the trouble of a second surgery.

Hello Everyone,

I have been suffering from hemorrhoids for probably 40+ years. I am 67. Deciding to finally do something about them, I went to my Gastroenterologist. I just finished my 4th rubber band ligation 2 days ago. The homorrhoids seem to keep forming! My doctor suggested doing some research on Transanal Hemorrhoidal Dearterialization (THD). Coming to this site for the first time, it appears that THD has been more positive than negative, so I made an appointment to be evaluated for that procedure next week.

Yesterday morning I awoke and the external hemorrhoids were the worst ever. It was a struggle just to walk. Wiping after a BM was quite painful. My gastro doc had told me to quit using  wet wipes and quit using Anucort but instead told me to use Balneol lotion for cleaning and then prescribed Hydrocortisone Acetate 2.5% Pramoxine HCL 1% cream to be applied daily as needed. These things did nothing to allieviate the pain, blood or itching.

I remembered something that had 1 worked for me in the past. This is the important thing i want to pass on to everyone. Yesterday morning about 10AM I took 2 Loperamide Hydrochloridl Tablets 2 MG which are meant to be used for diahrrea but work wonderfully for my current condition. In about 1 hour my pain started to subside. Yesterday afternoon and evening and night went without any pain or itching. The blood lessened to almost nothing. I could wipe myself without pain.  The next day (today) until 4PM i had no pain or itching.

I realize that you cannot continue to take anti-diahrrea medication for too long without stopping yourself up. I have been able to have normal BM's starting last night and into today.  I highly recommend this for anyone who is in constant pain and itching due to internal and/or external hemorrhoids. If my itching persists or worstens, i will take one pill tomorrow morning - in other words waiting a day inbetween doses.

One other thing for those of you who 'leak' blood or stool. I have started using thin Maxie Pads bought at Dollar General. They are easy to 'install' and work great.

I will reply again if I end up having that THD procedure.

I just had this surgery two days ago and I thought I was dying I was vomiting fever and intense pain and still no bowl movement and so much pressure from the plug... I was given Percocet for pain stool softener n x lax which I'd advise not to use because it made me sick and I had to call the doctor today for nausea pills n I'm able to final get up I'm running on little sleep I am 27 n 5 feet tall n only 128lbs so I'm handling none of this well at all... I don't know if I'd recommend the THD because of my experience but everyone is different. they woke me up after the surgery because they said I was hyperventilating and crying intensely because of the pain they gave me pain medication but nothing's stopping The pain and I did not get to sleep during my recovery

Hey GM,
I can't tell you how happy I was to come across your post. I recently had a doctor's exam as I have been bleeding with every bowel movement. The doctor suggested I have the THD procedure done as this would be the best option for me. I'm currently waiting for clearance from my Cardiologist. Thanks again for putting me at ease by sharing your experience.

Hello there, Can any of you recommend a very experienced surgeon with THD in any of the below countries: Switzerland, Italy or Germany. I am in Ghana and the procedure is currently not available here. I need to have the procedure done asap as it is beginning to take a toll on my quality of life. :) Thank you.

I am a 30 year old male with a history of hemorrhoids (several bindings since age 25) and one fissure (successful fistulotomy one year ago), otherwise healthy. This morning at 9AM I had THD performed on two large internal hemorrhoids (one stage 2, one stage 3, partially external and was becoming painful, hard to clean and annoying to push back in all the time). I was home by 3PM, and have had quite a bit of pressure and [manageable] pain so far. Pelvic area inflammation likely explains the slight difficulty in urination. It's 10PM and I'm posting from my phone in a hot bath. Thanks to everyone for sharing your experiences, and I wanted to reiterate what so many have already said: everyone's different... I passed gas over the toilet around 4PM, and the gauze/foam plug came right out. I've felt a constant urge defacate and tried a couple more times but was not successful until just recently (hence the bath). I'm optimistic that means I won't have the 7+ day, painful, constipated, recovery period I've seen described so much here. One thing I did not see commonly mentioned was the numbess of the rectum. I'm assuming a local anesthetic was used to reduce the immediate post-op pain, and hope that doesn't mean I'll feel more pain in the next couple days. Tips: Enema before procedure High fiber, laxatives and stool softeners before and after procedure (and a healthy diet from then on). Never strain. Your body knows how to move it's bowels - rushing the process causes problems. I'm convinced my problems are mainly due to straining a lot throughout my twenties). Ask your doctor how many of these performed, and research where you can. Being confident in your surgeon is important.

Day 2, just slept like a rock for 7 hours. Woke up and had a relatively painless BM. I had no dainage overnight, and the dressing was clean. The BM wasn't pleasant, but I have no painkillers in my system and it wasn't terrible. My rectum is still numb, so we'll see what happens when that wears off. I had a green smoothie, rice, chicken and vegetables yesterday. After this bath I'll go fire up the Vitamix. So far, no complaints, and it's nice not having a protrusion anymore!

Day 4, more of the same. BM's are less painful, and the majority of pain is following a BM, manageable with a painkiller. I can tell that things are in place, and I'm healing. I'll come back if anything significant happens, but I'm assuming the pain will slowly fade and I'll be good to go!

Did you have any protusion immediately after surgery. I had mine 4 days ago and I still do. Is that normal? I had internal and external.

Hello,

Very glad I had THD, even though it was NOT the 48-hour recovery predicted by my doctor and cited on the industry web sites.  More like two to three weeks.  As they say, everyone is different, and in my case I had "tenesmus" for two to three weeks -- the constant feeling of needing to "go."  The nerves in that area, having been disturbed, were basically fooling me into believing that I had to "go" all the time.  Also, in my case, bowel movements were painful for the first 10 to 14 days.

I am a long-haul international pilot, so I couldn't go to work in marginal condition.  When I complained of my situation to my doctor, he predicted all would be well after three weeks.  That turned out to be true.

I remain VERY happy that I had the THD, but I really wish the industry and the doctors would be more up-front about the highly variable individual experiences, instead of quoting the best-case outcome to everyone.

P.S.  For temporary relief of hemorrhoid pain/itching, I recommend just a dab of Desitin Cream -- baby diaper rash ointment.  It's a simple white zinc-oxide ointment in a tube, inexpensive, and very effective.  Glad I had it when I needed it.  Glad I don't need it any more.

Hi, I just got the THD procedure. Not so bad at all. A manageable pain. The bad thing is after defecation I have some bad moments and the pain is rising. I mean that after each pooping, the pain is getting worse. For example, now it is 15.15 hrs and I already got 3-4 defections. The THD was on Monday, 16th March. Does anyone know when these will going to stop? p.s. I am not a native English speaker, please excuse me for my not so good language.
