Recently, however, I had some serious back trouble and was diagnosed with a protruding L4/5 disc causing significant impingement of my thecal sac. It also seemed to me that during this time that my penis and now scrotum also were falling asleep in seated positions with greater regularity. This symptom, coupled with the growing numbness in my legs and accompanying neuralgia, had my neurologist scheduling me for immediate surgery. In the days before surgery I was beginning to notice a slight, spontaneous tingling in the underside of my scrotum which would come and go like goosebumps. I was noticing this in any position, though at this point I was spending 90% of my day flat on my back.
I have had back trouble all my life and I sort of told myself that the penile numbness I was feeling was related to this disc herniation which seemed to me had been developing for years. Now, while I have not experienced the penile and scrotal numbness since my surgery (though I haven't spent any appreciable amount of time seated), the tingling in my scrotum has returned after two days of not feeling it at all. Again, I spend most of my day on my back and am wondering if I'm putting pressure on those nerves, or if it's even neurologically related at all. I hate to think I've gone through this traumatic surgery for no reason.
So, anyone know what the root causes of penile and scrotal numbing in seated positions are, and what is indicated by mild tingling on the underside of the scrotum?

This may have nothing to do with your back problems.
How old are you? My father suffers from chronic prostatitis and he gets those tingling sensations in his testicles. Before he was diagnosed with prostatitis, he was tested for STD’s, which we all laughed about since he is asocial person and spends most of his time at home with my mom and we used to make fun of him but the doc insisted on checking up on STD’s, so you may consider this as well. Have you undergone any risky sexual activity before your tingling sensations started?
A urologist may want to check your urine or prostatic secretions in order to determine possible infections. He may also want to do a scrotal examination to check for anatomical abnormalities and see if maybe epididymitis had developed.
Naturally, during this back injury, my sexual activity has crawled to halt. I've been monogamous with my wife for well over nine years now so an STD is competely out of the question. In fact, this tingling comes on the heels of sexual inactivity. Since doing some more research I've begun thinking this could be related to prostatitis, so, remembering my MD's advice of 20 years ago, I've tried to have as many orgasms as possible in my debilitated, medicated state -- orgasm isn't so easy with a post-surgical back injury and opiates, but I've managed three in the last two days. Still tingling. Pretty regularly. My neurologist recommends that I wait it out a little longer to see if it rights itself, but when I'm referred back to my MD for PT, you better believe I'll be asking to see a urologist if it doesn't go away. It's not something that's affecting my quality of life (yet). It's just persistent and freaking me out.

My experience was first pins and needles of both buttocks, followed by both legs, then the scrotum now the penis... all this pins and needles only starts with prolonged lying on my back and sitting positions, accompanied with some pain at the low back at times. at times i also experience some slight burning sensation in my left upper lateral thigh... i went to the Doctor and had an mri of the back, showed bulge of L4-L5, but, my symptoms are way more than what a bulge at that level may present. Im in the medical field... and i know that my symptoms are consistent with either disc problems, or probably Multiple sclerosis... but if it is multiple sclerosis... why is it mostly present when im sitting or lying down... Its driving me crazy... any help or feedback will help...

I know your post is in turn now an old one but it is the first posting that I have come across on any forum that is so similar to my own condition. Mine was numbness of the whole saddle area around 2 years ago, spreading down my legs till I could only walk with the aid of a crutch. Pins and needles was an improvement for me.
After MRI scans, blood tests, physio etc, it was eventually diagnosed as Vitamin B12 deficiency or pernicious anaemia. With regular B12 injections (which I have to do for life), the legs are quite a lot better and I now walk at least 4km a day. However, having got a lot better on that front, I have been turning my attentions to the penile numbness that I have experienced throughout that period and consequent erectile dysfunction. My wife has been extremely understanding but I really want to solve this one. I'm 58 and don't want to think that this is never going to get better.
I'm happy to offer more of my own experiences if they might help. Have you been able to get a good diagnosis and find a solution?
I have the same tingling, numbness etc. in my scrotum. Have had it for years unfortunately. I've seen about every type of doctor there is to no avail. Lyrica helped but I hate that stuff. I too had/have a minor disk bulge but have been told its either not the disc that would affect that area or that the bulge is not enough to affect that area. My numbness usually happens after sitting (although rather infrequently). Let me know if anybody had found any relief. We may be our own best help when it comes to figuring this out.

Dr. though MS initially. Went to several neurologists and had several MRIs they cannot find anything wrong. They said 20% of patients with this numbness go undiagnosed. They recommend learning to handle stress better but no real help. I have been dealing with this for ~8 months. No relief in sight.

Sometimes it really is a circulation issue. Try using a first rate penis health crème with L-arginine, which helps boost nitric oxide (and which in turn helps with blood flow.)

one day i visited one place and found difficult in sitting on the floor, felt like contractures at pelvic area,but i sat up (that was new thing for me) and than one day while surfing net and sitting in same odd position i felt numbness in my penis , i immediately stood up and manipulated my penis in hand and sensitivity regained immediately but after a few days i developed numbness in my penis till now and i dont feel any taste in sex even. my penis and scrotum in numb and i feel moderately severe pain at the ring of penis (the junction of penis and skin, i am circumcised person). i have erections but not very strong that i had before. and i feel the taste coming from prostate only. i believe this is pudendal nerve issue because i was having pain at gluteal regoins while sitting that is resolved now. i was feeling that my gluteal muscles are gone and was feeling pain while sitting before. but now no pain and i am active with no exercise limitations.PLEASE I NEED ANY EXERCISE OR OTHER TREATMENT. I SHALL BE GRATEFUL SIR
My experience is 99% identical to yours. Started with pins and needles on my buttocks and legs, then burning/itchy sensations under scrotum, then all these was accompanied by Iower back pain, then I got numbness in perineum and penis. The MRI didn't show anything that would explain these symptoms. I was wandering if you were ever able to figure out what it was and how to cure it. I would really appreciate your reply.
