"BACK TO NORMAL" Am I the only man who thinks RE fundamentally different and totally less satisfying? The whole second half of the orgasm is gone. No calm feeling after. It feels just like I have had a shot of adrenalin afterwards. Maybe I was the only one who felt so relaxed after. It was just like taking a tranquilizer only natural and better. It's that feeling that make you want to go to sleep after. I just don't understand why I seem to be the only person who is noticing it is gone.
I am not sure why you are the only person noticing/reporting this. It is the 1st I have heard and I have read everything I could get my hands on.
What does your doctor say?
I am a HoLep graduate (Dr. Lingeman) and RE is basically the same for me only "dry". Occasionally a drop or two.
It sure beats self cath many times per day/night.

Those of you who have had the HoLEP procedure, can you please outline briefly what tests are done and what issues are explored in the first, pre-surgery visit to Mayo (either MN or AZ)? Is the visit a one-day event or longer? How long after the first visit is the surgery typically scheduled? Thanks.

My doctor was Dr. Humphries at the Mayo in Phoenix. He said he has never heard of it and that he called Dr Lingeman also and he had not heard about it as well. I have been to several physiologist and therapist as well as other types of doctors, Neurologist, Naturalpath, etc… and none of them think it can be caused by mental or emotional problems. It is a parasympathetic system response that releases the endorphins and other chemicals.
I am totally at a loss why it is happening. There are others on different forums that are experiencing it also. It is really making my life very unpleasant. Because I am so hyper all the time now and feel restless because I cannot get that calm. I wish I had never done this surgery. I should have looked harder at other procedures.
Marco, just call the Mayo and ask. I had 4 or 5 test prior to my surgery. A cystocopy, a urinflo dynamics study, prostrate ultrasound and blood work etc..
I have written to you before as a Guest-- maybe Dr. Humphries or Dr. Lingeman (these 2 are among the best) can contact their colleagues in New Zealand who developed the HoLep and inquire with them about your ongoing issues.
Again, I am pleased with the HoLep and especially with Dr. Lingeman.
I am not trying to make any "points" here- I am just trying to educate myself as to the HoLep.
One question I have is the "durability"--is it 10 plus years? or more? there is limited info.
10 years plus I was told. I wish it was less. As for the doctors looking into it more. I am sure they really don't care to spend to much time to figure out what is going on because they see it as a very small problem. But I am sure there are a lot of men that feel as I do that are not speaking up. Men are macho and don't like to talk about this stuff. But other websites about RE have a lot of unhappy men.
I just had a check-up with the local urologist following HoLep with Dr. Lingemen in Jan., 2013. A bladder scan indicated that there was zero residual (quite pleasing result).
The reason that there is RE is that when you remove the inside of the prostate capsule-- through any procedure--, you can not 'squeeze' that prostate channel when you ejaculate, so the sperm goes into the bladder (not out the urethra).
This well known and well liked local doctor struggled with understanding the HoLep procedure--they really need to be made more aware of the procedure. I am very pleased w. Dr. Lingeman.
I had GL laser procedure for BPH less than 3 weeks ago for a LARGE prostate. The day after the procedure I was peeing like a firehose! Urine sometimes has a pinkish tinge, and sometimes not. Very little pain - I have taken no pain meds following the procedure. Chose GL because anesthesia was not quite as profound as HOLEP laser required - real general anesthesia has some incidence of dementia and hallucinations in men over 65 so I ruled TURP out (also because of TURP-associated morbidity and possible complications). My doctor was one of the very first urologists in Arizona to use lasers for BPH and so has done MANY GL procedures. Since this IS a surgical procedure, I am surprised at how well I have tolerated recuperating, and how remarkably well my plumbing is working now. (When you have had BPH for a long time you grow used to peeing poorly and frequently.) I would recommend the GL laser for BPH to any friend or relative. If I continue to recuperate successfully, this surgery will have definitely changed my life for the better.
Yes it's great if you don't care about Retrograde Ejaculation after. I don't think it is a god trade off.
Well, an unfortunate update on Dr. Richard David - everything seemed to be going very well at the date of my last post above. Unfortunately, I ended up in the ER with massive blood clots blocking my urethra just days after making the above post. Apparently, Dr. David had left residual tissue in the prostate or failed to properly cauterize blood vessels or something. At this point, I am going to be obtaining medical records to try and find out what happened. My biggest issue with Dr. David is that he effectively "abandoned" me during the emergency crisis - I began experiencing very large blood clots coming out of my urethra after having driven from Long Beach, California to Tucson, Arizona. I called his office and was only able to reach the urologist on duty who then called Dr. David with my symptoms. Incredibly, I was simply advised not to worry about it even after advising the on call urologist (and therefore Dr. David) that I was planning to drive into Mexico. I drove one day into Mexico and immediately returned the next day because of literally sheer fear of being caught in an emergency situation there. Smartest move I could have made - once out of Mexico and on a plane flight back to Long Beach (I left my vehicle in Arizona because I did not think I could make it back driving). Before the plane had even landed at Long Beach airport, I was on the flight attendant's cell phone (I was so fearful and upset that I accidentally left my cell phone in Arizona) calling my son and my wife to see who was picking me up and tell them that we were heading straight to the emergency room. I literally walked off the plane, did not even bother to pick up my luggage (the pain was so great that I knew I didn't have much time), got into the car and went straight to the ER at Los Alamitos Medical Center. I was in the hospital for two days and lost almost half my blood volume as a result of the internal prostate bleeding - literally a terrifying life-threatening experience unlike anything I have ever experienced before!
At this point in time, I cannot make any determinations as to what went wrong (or Dr. David's culpability) but I can definitely make a statement about a doctor failing to provide follow up and support for a patient who suffered a major complication as a result of his (Dr. David's) professional work. When I was in Mexico (after I had personally called Dr. David from Arizona regarding the problem), my wife had to call him again on my behalf. As a Spanish speaker, Dr. David could have provided literally life-saving support in the event I had been forced to enter the ER in Mexico. Unfortunately, there was no offer of any support whatsoever on his part, no follow up call of any kind to see how I was doing, and I have never heard from him again either before or after entering the ER in Los Alamitos. I have since connected with another urologist (Dr. Jennifer Liu of Lakewood, California) who performed a secondary TURP to remove the tissue left behind by Dr. David and also cauterize additional blood vessels - I am now almost 4 weeks after the TURP procedure and everything seems to be resolved. Dr. Liu is a much younger (and supposedly less experienced) doctor than Dr. David professed to be but she is very sharp and VERY compassionate - she ALWAYS returned my phone calls literally ASAP, always provided very believable guidance and reassurance about whatever was happening with me post-TURP, and was even returning my calls after I had moved to Las Vegas. BTW, Dr. Liu is one of the HOLEP urologists that I consulted with prior to selecting Dr. David for the procedure - I chose Dr. David over Dr. Liu because he professed to have 15 years of experience and Dr. Liu is only 8 years out of medical school. I believe now that I made a big mistake and should have gone with Dr. Liu.
But the bottom line is - you DO NOT want a doctor who is going to fail to take care of his patients when complications occur. I am very fortunate that this was not a fatal event for me in Mexico as even the ER personnel in the Los Alamitos Medical Center did not know how to treat my problem. I was very fortunate that the urologist on call that night at Los Alamitos showed up and corrected the situation by placing me on continuous bladder irrigation (it is highly likely I would have not been that fortunate in a Mexican ER). I am not sure of Dr. David's potential legal or professional liability for this act of patient "abandonment" but will definitely be filing a complaint with the California Medical Board. Given the life-threatening severity of what happened, I can no longer in any manner make any positive recommendation of Dr. David - given what happened and his response to it, I find it hard to believe that he possesses the experience and skills professed and, even if he did, that would not make up for the apparent utter lack of caring and/or post-operative support. I cannot in good faith make any positive recommendation of this urologist and must withdraw the above post in its entirety.

I also had Holep surgery at the Mayo Clinic with Dr Humohreys on September 26, 2013. I also experience no pain, no blood, no leakage, great flow, and sleep through the night.
What I learned from this web site and others is to stick with the large institutions (Indiana Univ.-Dr. Lingeman) and the Mayo Clinic.
THey have the capacity/expertise to deal with major issues that arise from this surgical procedure.
